What Is Immunotherapy?

Immunotherapy is a modern medical treatment concept in which the body's own immune system is deliberately influenced to fight disease or reduce its severity. Unlike conventional treatments such as chemotherapy or radiation therapy, immunotherapy harnesses the natural defense mechanisms of the body. It is widely used today in oncology (cancer medicine), as well as in the management of allergies, autoimmune diseases, and chronic inflammatory conditions.

Types of Immunotherapy

Checkpoint Inhibitors

Checkpoint inhibitors are antibodies that release certain braking mechanisms of the immune system. Tumor cells exploit these brakes to hide from immune surveillance. By blocking proteins such as PD-1, PD-L1, or CTLA-4, the immune system is re-enabled to recognize and attack cancer cells.

CAR T-Cell Therapy

In CAR T-cell therapy, the patient's own T cells (a type of white blood cell) are collected, genetically engineered in the laboratory to specifically recognize cancer cells, and then reinfused into the body. This therapy has shown particularly impressive results in certain types of blood cancer.

Monoclonal Antibodies

Monoclonal antibodies are laboratory-produced proteins that bind selectively to specific structures on tumor cells or immune cells. They can directly destroy cancer cells, inhibit tumor growth, or mobilize the immune system against the tumor.

Cancer Vaccines (Therapeutic Vaccines)

Therapeutic cancer vaccines stimulate the immune system to recognize tumor antigens (tumor-specific surface markers) and mount a targeted immune response. They are used to treat existing tumors and are distinct from preventive vaccines.

Cytokine Therapy

Cytokines are signaling molecules of the immune system. In immunotherapy, cytokines such as interferon or interleukin-2 are administered to enhance the activity of immune cells, thereby improving the defense against tumor cells or viruses.

Allergen Immunotherapy (Desensitization)

Allergen immunotherapy, also known as desensitization or allergy shots, is the only causal treatment for allergies. The patient is exposed to gradually increasing doses of the triggering allergen to develop tolerance. It is used for hay fever, insect venom allergy, and house dust mite allergy.

  • Oncology: Melanoma, lung cancer, bladder cancer, leukemia, lymphomas, and many other tumor types
  • Allergology: Pollen allergy, insect venom allergy, house dust mite allergy
  • Autoimmune diseases: Regulation of excessive immune responses
  • Infectious diseases: e.g., chronic hepatitis, HIV (under investigation)

How Does Immunotherapy Work?

The core principle of immunotherapy is to alter the balance of the immune system. Depending on the disease, this can mean:

  • Activation: The immune system is boosted to better combat tumor cells or pathogens.
  • Suppression: In autoimmune diseases or organ transplant rejection, the immune system is deliberately dampened.
  • Modulation: The immune system is reprogrammed to stop overreacting to specific substances, such as allergens.

Side Effects

Immunotherapies can cause various side effects depending on the method used. Because the immune system is active throughout the entire body, so-called immune-related adverse events can occur in multiple organs:

  • Skin reactions (rash, itching)
  • Fatigue and general weakness
  • Inflammation of the lungs, liver, intestines, or thyroid
  • Flu-like symptoms (with cytokine therapies)
  • Severe allergic reactions (rare)

Most side effects are manageable when detected early. In cases of severe immune-related reactions from checkpoint inhibitors, corticosteroids are often used as treatment.

Diagnosis and Treatment Planning

Before initiating immunotherapy, physicians conduct comprehensive diagnostics. This includes molecular biological tests to determine whether a tumor carries specific characteristics that make immunotherapy promising -- such as PD-L1 expression or microsatellite instability (MSI). Treatment decisions are typically made within an interdisciplinary tumor board.

Current Developments and Research

Immunotherapy is one of the fastest-growing fields in medical research. Current studies are investigating combination therapies (e.g., checkpoint inhibitors plus chemotherapy), personalized cancer vaccines based on individual tumor mutations, and novel CAR T-cell constructs for solid tumors. Advances in this field have significantly improved survival rates for many cancer patients in recent years.
