What is the Impressio cardiaca?

The impressio cardiaca (Latin for cardiac impression or heart indentation) refers to an anatomical depression formed by the pressure of the heart on adjacent organs. It is a completely normal finding and not a sign of disease. Depending on the organ affected, a distinction is made between the impressio cardiaca pulmonis (on the lung) and the impressio cardiaca hepatis (on the liver).

Anatomical Location and Occurrence

Impressio cardiaca pulmonis

The most common form is the impressio cardiaca pulmonis. It is located on the medial (midline-facing) surface of the left lung, where the heart lies in direct contact. The weight and pressure of the heart creates a characteristic indentation in this area during development. This depression is clearly visible on anatomical specimens and in medical imaging and is part of the normal anatomy of the lung.

Impressio cardiaca hepatis

On the liver, the impressio cardiaca hepatis is found on the diaphragmatic surface of the right lobe of the liver. Here, the heart presses through the diaphragm onto the upper surface of the liver, also leaving a shallow indentation. This finding is equally physiological and has no pathological significance.

Clinical Relevance

In most cases, the impressio cardiaca has no clinical relevance whatsoever. It is frequently discovered as an incidental finding during imaging studies such as chest X-ray, computed tomography (CT), or ultrasound. Radiologists and physicians describe it in their reports as a normal anatomical structure.

However, in rare cases, an unusually pronounced or enlarged cardiac impression may indicate an enlarged heart (cardiomegaly) or increased pressure within the thoracic cavity. In such situations, further cardiological evaluation is recommended.

Appearance in Medical Imaging

In medical imaging, the impressio cardiaca appears as a smooth, arc-shaped indentation on the surface of the corresponding organ. Key features that physicians assess when evaluating this finding include:

  • Smooth, regular border of the indentation (consistent with normal anatomy)
  • No mass lesion or tissue consolidation in the surrounding area
  • Symmetrical position corresponding to the location of the heart
  • No displacement of adjacent structures

Irregular, sharply demarcated, or asymmetrical findings should be further investigated, as they may indicate other causes such as tumors, cysts, or adhesions.

Differentiation from Pathological Findings

Although the impressio cardiaca itself is benign, it must be distinguished from similar-appearing but pathological changes. These include:

  • Space-occupying lesions (e.g., tumors or cysts) pressing on the lung or liver from outside
  • Pleural effusion or pericardial effusion, which can alter the contours of the heart and neighboring organs
  • Adhesions resulting from inflammation or prior surgery

An experienced radiologist or physician can usually distinguish these conditions reliably based on imaging morphology and clinical information.
