Definition and Anatomy

The incisura acetabuli (Latin: notch of the acetabulum) is a natural, crescent-shaped gap located at the inferior margin of the acetabulum – the cup-shaped socket of the pelvis that forms the hip joint. The acetabulum is the bony cavity in which the spherical head of the femur (femoral head) sits, together forming the hip joint. The incisura acetabuli represents a physiological, naturally occurring interruption in the bony rim of the acetabulum.

This notch is bridged by the transverse acetabular ligament, which closes the gap in the cartilaginous rim and completes the acetabular ring. The resulting canal allows blood vessels and nerve branches to enter the interior of the hip joint, supplying the femoral head and joint structures.

Function

The incisura acetabuli serves several important functional roles in the human body:

  • Vascular passage: Branches of the obturator artery pass through the notch, supplying the ligament of the femoral head and the femoral head itself with blood.
  • Neural supply: Small nerve branches that innervate the joint capsule and internal joint structures also pass through this region.
  • Joint stability: The bridging transverse acetabular ligament contributes to the stability of the hip joint and completes the cartilaginous rim known as the acetabular labrum.

Clinical Relevance

In clinical practice, the incisura acetabuli is particularly important in imaging diagnostics and during surgical procedures involving the hip joint.

Imaging and Radiology

On pelvic X-rays, the incisura acetabuli appears as a characteristic indentation at the inferior acetabular margin. It serves as an anatomical landmark for radiologists when assessing the acetabulum. Changes in its shape or size may indicate pathological processes, such as hip dysplasia (abnormal development of the hip socket) or degenerative conditions like coxarthrosis (hip osteoarthritis).

Hip Arthroscopy

During minimally invasive hip procedures (arthroscopy), precise knowledge of the incisura acetabuli and the transverse acetabular ligament is essential to avoid injuring blood vessels and nerves in this area. Procedures involving the acetabular labrum require particular care in the region of the notch.

Hip Dysplasia and Impingement

In hip dysplasia, an altered shape of the incisura acetabuli may indicate insufficient coverage of the femoral head. In femoroacetabular impingement (FAI), a condition in which mechanical conflict occurs between the femoral head and the acetabular rim, anatomical variants in the acetabular region play an important role.

Pediatric Orthopedics

In children, the incisura acetabuli is a key landmark for evaluating hip development, for example in the diagnosis of hip dislocation (developmental dysplasia of the hip) or Legg-Calve-Perthes disease.
