Inflammatory markers in the blood are laboratory parameters used to detect whether an inflammatory response is occurring in the body. They are measured through a blood test and provide physicians with important clues about acute or chronic inflammation, infections, autoimmune diseases, or tissue damage. The most important inflammatory markers include C-reactive protein (CRP), the erythrocyte sedimentation rate (ESR), the white blood cell count (leukocytes), interleukin-6 (IL-6), and procalcitonin (PCT).
CRP is one of the most sensitive and commonly measured inflammatory markers. It is produced in the liver and rises sharply within a few hours of the onset of inflammation. Normal value: below 5 mg/l. Elevated CRP levels can indicate bacterial infections, inflammatory conditions such as rheumatoid arthritis, or tissue damage.
The ESR measures how quickly red blood cells settle at the bottom of a test tube. An elevated value suggests inflammation, but it is less specific than CRP. The ESR responds more slowly and is therefore better suited for monitoring the progression of chronic diseases. Normal values: men up to 15 mm/h, women up to 20 mm/h (after the first hour).
The white blood cell count provides information about immune system activity. An elevated count (leukocytosis) is often a sign of bacterial infection, while a low count (leukopenia) can indicate viral infections or immune deficiency. Normal range: 4,000 to 10,000 cells per microliter of blood.
Interleukin-6 is an immune signaling molecule (cytokine) released during inflammation that stimulates the liver to produce CRP. IL-6 rises very early in the course of an inflammatory event and is especially useful in severe infections or for monitoring critically ill patients.
Procalcitonin is a specific marker for bacterial infections and sepsis. It typically remains low during viral infections, making it a valuable tool for distinguishing between bacterial and viral causes. The normal value is below 0.5 ng/ml. Significantly elevated PCT levels can indicate life-threatening sepsis.
Inflammatory markers are measured through a straightforward blood draw, typically from a vein in the arm. The blood is then analyzed in a laboratory. Measurements are usually part of a complete blood count or a specific inflammation panel. Elevated values alone are not a definitive diagnosis – they must always be interpreted in the context of the patient's symptoms and additional diagnostic findings.
Treatment is guided not by the laboratory values alone, but by the underlying cause of the inflammation:
Mildly elevated inflammatory markers without clear symptoms should be evaluated by a physician and monitored closely to detect chronic inflammation at an early stage.
Anyone experiencing persistent symptoms such as fever, pain, fatigue, or swelling should consult a doctor promptly. If routine blood tests reveal elevated inflammatory markers, a medical assessment is equally important.
