What Is Integrated Care?

Integrated care is a healthcare model that aims to coordinate services across different sectors – including primary care, specialist care, hospital services, rehabilitation, and social care – to provide seamless, patient-centred treatment. Rather than treating each medical encounter in isolation, integrated care focuses on continuity, communication, and collaboration among all healthcare providers involved in a patient´s journey.

The concept has gained significant attention globally as healthcare systems face increasing pressure from ageing populations, rising rates of chronic disease, and fragmented service delivery. In Germany, integrated care is legally anchored in the Social Code Book V (SGB V), particularly under § 140a SGB V, which allows statutory health insurers to conclude contracts with multiple providers outside standard care structures.

Goals of Integrated Care

Integrated care pursues several key objectives:

  • Improving the quality and continuity of care through coordinated, guideline-based treatment pathways
  • Reducing duplication of tests and procedures
  • Preventing avoidable hospital admissions through stronger outpatient structures
  • Enhancing patient empowerment and self-management

Key Models and Structures

GP-Centred Care

GP-centred care (Hausarztzentrierte Versorgung, HZV) is one of the most established integrated care models in Germany. In this model, the general practitioner (GP) acts as a central coordinator. Patients who voluntarily enrol in such a programme commit to consulting their GP as the first point of contact before seeing a specialist.

Disease Management Programmes (DMPs)

Disease Management Programmes (DMPs) are structured, evidence-based treatment programmes for patients with chronic conditions such as type 2 diabetes, coronary heart disease, or bronchial asthma. They link outpatient and, where necessary, inpatient care through standardised treatment protocols and regular follow-up appointments.

Medical Care Centres (MVZ)

Medical Care Centres (Medizinische Versorgungszentren, MVZ) are outpatient facilities where physicians from different specialties work together under one roof. They promote interdisciplinary collaboration and simplify cross-sector coordination for patients with complex needs.

Selective Contracts under § 140a SGB V

Under § 140a SGB V, statutory health insurers in Germany can enter into selective contracts with one or more healthcare providers to create tailored integrated care models. These contracts allow for customised care pathways that may differ from standard reimbursement rules.

Stakeholders Involved

Integrated care brings together a wide range of actors:

  • Statutory health insurers (GKV) as payers and contracting partners
  • GPs and specialists as outpatient providers
  • Hospitals and clinics for inpatient treatment
  • Rehabilitation facilities for post-acute care
  • Nursing and social care services for long-term support
  • Pharmacies and therapists (e.g. physiotherapists, psychotherapists)

From a patient perspective, integrated care offers several practical advantages:

Challenges and Criticism

Despite its benefits, integrated care also faces significant challenges. The fragmentation of healthcare systems across different sectors and funding streams makes widespread implementation difficult. Effective integrated care requires robust digital infrastructure – such as electronic health records (EHRs) – as well as strong commitment from all stakeholders. Data privacy concerns and conflicting economic interests among providers can further hinder collaboration.

International Perspective

Integrated care is a priority in health policy worldwide. The World Health Organization (WHO) promotes people-centred and integrated health services as a core strategy for strengthening health systems globally. Many countries, including the United Kingdom (NHS), the Netherlands, and the Nordic nations, have developed national integrated care frameworks that serve as international benchmarks.
