What Does Interfollicular Mean?

Interfollicular is a medical term derived from the Latin prefix inter- (meaning between) and the word follicle. It describes the anatomical region situated between individual follicles. Depending on the medical context, the term can refer to two different types of structures: hair follicles in the skin, or lymphoid follicles in lymphatic organs such as lymph nodes, the spleen, or tonsils.

Use in Dermatology

In dermatology (the medical field concerned with skin), interfollicular refers to the skin tissue located between individual hair follicles. Hair follicles are pocket-like invaginations of the skin from which hairs grow. The interfollicular epithelium is the epidermis (outer skin layer) that surrounds and connects these follicles.

This term has clinical relevance in several contexts:

  • Inflammatory skin diseases: Inflammatory cells may accumulate in the interfollicular tissue, which is important in the histological diagnosis of conditions such as psoriasis or eczema.
  • Wound healing and skin grafting: Interfollicular keratinocytes (skin cells) play a key role in the regeneration of the skin surface.
  • Skin biopsies: Pathologists assess the interfollicular epidermis to identify changes in tissue patterns.

Use in Pathology and Haematology

In pathology and haematology (the study of blood and lymphatic diseases), interfollicular refers to the tissue between lymphoid follicles within lymphatic organs. Lymphoid follicles are small, spherical clusters of immune cells (predominantly B-lymphocytes) that form part of the immune system.

The interfollicular tissue consists mainly of T-lymphocytes and other immune cells. It is an important area in the assessment of:

  • Lymphomas: Certain types of lymphoma, such as interfollicular Hodgkin lymphoma or interfollicular growth patterns in non-Hodgkin lymphomas, are characterised by the spread of tumour cells within the interfollicular region.
  • Reactive lymph node changes: During infections or inflammatory reactions, the interfollicular tissue may appear enlarged, which helps differentiate between benign and malignant changes.
  • Immunohistochemical studies: Analysis of the interfollicular region provides important information about the composition of immune cells and supports accurate diagnosis.

Interfollicular Growth Pattern in Lymphomas

A particularly relevant clinical concept is the interfollicular growth pattern in lymphomas. In this pattern, tumour cells spread preferentially through the tissue between preserved lymphoid follicles without initially destroying them. This pattern can make diagnosis more challenging, as the follicular architecture may appear intact at first glance. Careful histological and immunohistochemical examination is therefore essential.

Diagnostic Significance

The term interfollicular is especially important in histopathology (the microscopic examination of tissue). Pathologists use it to precisely describe the location of cellular changes or inflammatory reactions within a tissue sample. This allows for accurate diagnosis and helps determine the most appropriate treatment.
