What is Interleukin-2 Therapy?
Interleukin-2 therapy (short: IL-2 therapy) is a form of immunotherapy that stimulates the body's own immune system to recognize and destroy cancer cells. Interleukin-2 is a naturally occurring cytokine – a signaling molecule of the immune system – that promotes the proliferation and activation of T-lymphocytes (white blood cells). These cells play a central role in fighting tumors.
Since its approval by the U.S. Food and Drug Administration (FDA) in the 1990s, high-dose IL-2 therapy has been one of the oldest approved immunotherapies for cancer.
Mechanism of Action
Interleukin-2 binds to specific IL-2 receptors on the surface of immune cells, triggering a signaling cascade that leads to the activation and proliferation of:
- Cytotoxic T cells (CD8+ T cells), which directly kill tumor cells
- Natural killer cells (NK cells), which recognize and eliminate cancer cells
- Regulatory T cells (Tregs), which modulate the immune response
The therapeutic administration of recombinant IL-2 (artificially produced IL-2) greatly increases the concentration of this signaling molecule in the body, stimulating an intense immune response against tumor cells.
Indications
IL-2 therapy is primarily used for the following conditions:
- Metastatic renal cell carcinoma (advanced kidney cancer)
- Metastatic melanoma (advanced malignant skin cancer)
In clinical trials, IL-2 therapy is also being investigated in combination with other immunotherapies, checkpoint inhibitors, or targeted therapies.
Dosage and Administration
IL-2 therapy can be administered in different dosing regimens:
- High-dose IL-2 (HD-IL-2): Intravenous administration at high doses in specialized centers; requires intensive medical monitoring
- Low-dose IL-2: Subcutaneous injection (under the skin); better tolerated but less effective
High-dose therapy is only performed in specialized oncology centers due to the potential for serious side effects.
Side Effects
IL-2 therapy can cause significant side effects, especially with high-dose treatment:
- Capillary leak syndrome: Fluid leakage from blood vessels, which can lead to edema and low blood pressure
- High fever, chills, and general malaise
- Nausea, vomiting, and diarrhea
- Exhaustion and weakness (fatigue)
- Skin reactions such as redness and itching
- In severe cases: cardiovascular problems and kidney failure
Due to these risks, careful patient selection and medical supervision are essential.
Treatment Context and Current Developments
With the development of newer immunotherapies such as checkpoint inhibitors (e.g., PD-1/PD-L1 inhibitors), high-dose IL-2 therapy has become less prominent, as these agents are often better tolerated. Nevertheless, IL-2 therapy remains an option for selected patients in good general health, as it can lead to durable remissions in a subset of patients.
Current research focuses on modified IL-2 variants (so-called IL-2 muteins or IL-2 fusion proteins) that promise improved efficacy with fewer side effects.
