What is Interleukin-6?

Interleukin-6 (IL-6) is a cytokine – a small signaling protein that mediates communication between cells of the immune system. It is produced by a wide range of cell types, including macrophages, T cells, fibroblasts, and endothelial cells. IL-6 plays a central role in orchestrating inflammatory responses, immune defense, and a variety of metabolic processes throughout the body.

Biological Functions

IL-6 performs numerous important functions in the human body:

Acute-phase response: IL-6 stimulates the liver to produce acute-phase proteins such as C-reactive protein (CRP), which serve as markers of inflammation.
Immune regulation: It promotes the maturation and activation of B cells and T cells, supporting the adaptive immune response.
Hematopoiesis: IL-6 influences blood cell production, particularly the generation of platelets (thrombocytes).
Bone and cartilage metabolism: It regulates bone resorption and, when chronically elevated, can contribute to bone loss.
Metabolic effects: IL-6 modulates insulin sensitivity, energy metabolism, and fat oxidation.

Clinical Significance

Persistently elevated IL-6 levels in the blood are associated with a wide range of diseases:

Rheumatoid arthritis: IL-6 drives joint inflammation and contributes to structural joint damage.
Crohn's disease and ulcerative colitis: Chronic inflammatory bowel diseases in which IL-6 plays a key role.
Systemic lupus erythematosus (SLE): An autoimmune disease characterized by excessive IL-6 production.
Sepsis and cytokine storm: In life-threatening infections, an uncontrolled surge of IL-6 can trigger a cytokine storm, leading to multi-organ failure.
Cancer: Various tumor types produce IL-6, which can promote tumor growth and metastasis.
COVID-19: Severe cases were associated with a dramatic rise in IL-6 levels, contributing to pneumonia and organ damage.

Diagnosis and Laboratory Values

IL-6 can be measured in blood serum. Elevated values indicate acute or chronic inflammatory processes. In healthy adults, the reference range is generally below 7 pg/ml, though exact thresholds may vary by laboratory. IL-6 measurement is often used alongside other inflammatory markers such as CRP and procalcitonin for a comprehensive assessment.

Therapeutic Approaches: IL-6 Inhibitors

Because IL-6 is a key driver of many diseases, targeted medications have been developed to inhibit the IL-6 signaling pathway:

Tocilizumab (Actemra®): A monoclonal antibody that blocks the IL-6 receptor. It is approved for the treatment of rheumatoid arthritis, juvenile idiopathic arthritis, and severe COVID-19.
Sarilumab (Kevzara®): Also an IL-6 receptor antagonist, approved for rheumatoid arthritis.
Siltuximab (Sylvant®): Binds directly to IL-6 and is used in the treatment of Castleman disease.

These biologic medications have significantly improved outcomes in autoimmune diseases but may increase the risk of infections and require close medical monitoring.

IL-6 and Exercise

Interestingly, IL-6 is also released by muscle cells during physical activity. In this context, it does not act primarily as a pro-inflammatory signal but rather as a metabolic regulator: it enhances fat oxidation, improves insulin sensitivity, and supports energy balance. This muscle-derived form of IL-6 is classified as a myokine.
