What is Isotretinoin?

Isotretinoin is a synthetic retinoid chemically related to vitamin A. It is available under several brand names and is considered one of the most powerful treatments for severe acne vulgaris. It is available by prescription only and is typically reserved for patients who have not responded adequately to other therapies such as antibiotics or topical treatments.

Indications

Isotretinoin is approved for the treatment of:

  • Severe nodulo-cystic acne that has not responded to conventional treatments
  • Acne with a high risk of permanent scarring
  • Acne causing significant psychological distress

Mechanism of Action

Isotretinoin works through multiple complementary mechanisms, which distinguishes it from other acne medications:

  • Reduction of sebum production: It significantly inhibits the activity of sebaceous glands, decreasing the amount of skin oil (sebum) produced – a key driver of acne development.
  • Normalization of keratinization: It regulates abnormal skin cell turnover (hyperkeratosis) in the pores, which helps prevent blockages.
  • Indirect antibacterial effect: By reducing sebum flow, isotretinoin indirectly suppresses the growth of the bacterium Cutibacterium acnes (formerly Propionibacterium acnes).
  • Anti-inflammatory properties: Isotretinoin also exerts direct anti-inflammatory effects on the skin.

Dosage and Administration

Isotretinoin is taken as an oral capsule and should always be taken with a fatty meal to improve absorption. The dosage is determined individually by the prescribing physician:

  • Starting dose: 0.5 mg per kg of body weight per day
  • Maintenance dose: 0.5–1.0 mg per kg of body weight per day
  • Treatment duration: Typically 16–24 weeks per treatment course
  • Cumulative target dose: 120–150 mg per kg of body weight

A single course of isotretinoin leads to long-term or permanent remission of acne in many patients.

Side Effects

Isotretinoin can cause a range of side effects, some of which can be serious:

Common Side Effects

  • Dry skin, lips, and mucous membranes
  • Dry eyes and increased sensitivity to light
  • Dry nasal passages, occasional nosebleeds
  • Elevated liver enzymes and blood lipids (triglycerides, cholesterol)
  • Muscle and joint pain

Serious Side Effects

  • Teratogenicity: Isotretinoin is severely harmful to unborn children. It must never be used during pregnancy, as it can cause major fetal malformations.
  • Psychiatric effects: In rare cases, depression, anxiety, and suicidal thoughts have been reported. Patients and caregivers should monitor for mood changes throughout treatment.
  • Liver toxicity: Rare but possible; regular blood tests are required during treatment.

Pregnancy Prevention Programme

Due to the extremely high risk to the unborn child, isotretinoin is subject to a mandatory Pregnancy Prevention Programme (PPP) in Europe and similar risk management programmes in other countries. Requirements include:

  • Women of childbearing potential must use effective contraception before, during, and for at least one month after treatment.
  • Regular pregnancy tests are mandatory throughout the treatment period.
  • Prescriptions are limited to a maximum of 30 days and must be renewed monthly.

Contraindications

  • Pregnancy and breastfeeding
  • Severe liver or kidney disease
  • Uncontrolled elevated blood lipid levels (hyperlipidemia)
  • Hypersensitivity to retinoids
  • Concurrent use of tetracycline antibiotics (risk of increased intracranial pressure)
