What is KIM1?

KIM1 (Kidney Injury Molecule-1, also written as Kidney Injury Molecule 1) is a transmembrane glycoprotein expressed on the surface of proximal tubular cells in the kidney. Under normal physiological conditions, KIM1 is barely detectable in healthy kidney tissue. However, when the proximal tubules are damaged – as occurs in acute kidney injury (AKI) – KIM1 expression is strongly upregulated and the protein is shed into the urine. This makes KIM1 one of the most important early-detection biomarkers for kidney damage.

Biological Function

KIM1 belongs to the family of type 1 transmembrane proteins and is encoded by the HAVCR1 gene (Hepatitis A Virus Cellular Receptor 1). It plays a critical role in the recognition and clearance of dead or dying cells (apoptotic and necrotic cells) through a process known as phagocytosis. By performing this function, KIM1 supports renal tissue regeneration following injury. At the same time, sustained overexpression of KIM1 may promote fibrosis (scarring) and contribute to the progression of chronic kidney disease.

Clinical Relevance as a Biomarker

Acute Kidney Injury (AKI)

In cases of acute kidney injury, urinary KIM1 levels rise within hours of the damaging event – significantly earlier than the traditional marker serum creatinine. This makes KIM1 a valuable tool for early detection of kidney damage, particularly in the following scenarios:

  • Ischemia-induced kidney injury (e.g., following surgery or shock)
  • Nephrotoxic injury caused by medications (e.g., cisplatin, aminoglycosides, contrast agents)
  • Sepsis-associated nephropathy

Chronic Kidney Disease (CKD)

KIM1 also demonstrates prognostic value in chronic kidney disease (CKD). Elevated urinary KIM1 levels are associated with faster progression of kidney failure and an increased risk of cardiovascular events.

Diabetic Nephropathy

In patients with diabetes mellitus, elevated urinary KIM1 concentrations may indicate diabetic nephropathy even before albuminuria becomes apparent. This offers an additional diagnostic opportunity for early identification of kidney complications in diabetic patients.

Diagnosis and Measurement

KIM1 is typically measured from a urine sample, as the protein is shed from damaged tubular cells into the urinary tract. Measurement is commonly performed using an ELISA (Enzyme-Linked Immunosorbent Assay) or comparable immunological testing methods. To account for variations in urine concentration, measured KIM1 levels are often normalized to urinary creatinine (KIM1-to-creatinine ratio).

Reference ranges vary by laboratory and testing method. Clinically relevant elevations are generally well above the values observed in healthy control individuals.

Advantages Over Conventional Kidney Markers

Conventional kidney markers such as serum creatinine and blood urea nitrogen only rise detectably after a substantial loss of kidney function has already occurred. KIM1, in contrast, enables earlier detection because it directly reflects tubular cell damage and is independent of the glomerular filtration rate (GFR). As such, KIM1 represents a valuable addition to existing kidney biomarker panels.

Research and Future Perspectives

KIM1 is being actively investigated as a therapeutic target. Preclinical and clinical studies are exploring whether inhibiting KIM1 signaling pathways can slow renal fibrosis and halt the progression of chronic kidney disease. Additionally, KIM1 is increasingly used as a safety biomarker to detect nephrotoxic effects of novel drugs during clinical development.
