What is Klotho?

Klotho is an endogenous protein first discovered in 1997 by Japanese researchers. Its name is derived from Klotho, one of the three Fates in Greek mythology – the goddess who spins the thread of life. This reflects the central importance of the protein for lifespan and aging. Klotho is produced mainly in the kidneys, the brain (choroid plexus), and the parathyroid gland, and it exists in several distinct forms.

Forms of Klotho

  • Membrane-bound Klotho (m-Klotho): This form is anchored to the cell surface and acts as a co-receptor for Fibroblast Growth Factor 23 (FGF23).
  • Secreted Klotho (s-Klotho): This form is released into the bloodstream, urine, and cerebrospinal fluid, functioning as a circulating hormone with systemic effects.
  • Beta-Klotho: A related variant expressed primarily in the liver and adipose tissue, involved in metabolic regulation.

Mechanism of Action

Klotho exerts its effects through several mechanisms:

  • FGF23 Co-receptor: Membrane-bound Klotho is required for FGF23 to bind its receptor. Together, they regulate calcium and phosphate homeostasis as well as vitamin D metabolism.
  • Antioxidant Protection: Klotho shields cells from oxidative stress by enhancing the activity of antioxidant enzymes.
  • Modulation of Insulin Signaling: Klotho modulates the insulin and IGF-1 (Insulin-like Growth Factor 1) signaling pathways, which has been associated with extended lifespan in animal models.
  • Anti-inflammatory Effects: The protein reduces pro-inflammatory signaling pathways, protecting against chronic inflammation.
  • Kidney Protection: Klotho supports kidney health by inhibiting fibrotic processes and preserving renal function.

Relevance to Health

A deficiency in Klotho is associated with numerous age-related diseases. Animal studies showed that mice lacking a functional Klotho gene aged rapidly and died prematurely, while overexpression of Klotho extended their lifespan. In humans, Klotho levels naturally decline with age.

Conditions Associated with Low Klotho Levels

  • Chronic Kidney Disease (CKD): Low Klotho levels serve as an early biomarker and contributing factor in renal insufficiency.
  • Cardiovascular Disease: Klotho deficiency promotes arteriosclerosis and cardiac dysfunction.
  • Osteoporosis: The protein influences bone metabolism and helps protect against bone loss.
  • Cognitive Impairment and Dementia: Studies link lower Klotho levels with an increased risk of Alzheimer's disease and cognitive decline.
  • Diabetes Mellitus: Klotho is involved in insulin sensitivity and glucose metabolism.

Diagnosis and Measurement

Klotho levels can be measured in blood (serum), urine, and cerebrospinal fluid. Standardized reference values are still being established through ongoing research. In clinical practice, Klotho measurement is primarily used in scientific studies and within the field of nephrology. ELISA-based tests (Enzyme-linked Immunosorbent Assay) are the most commonly used measurement methods.

Ways to Support Klotho Levels

Various lifestyle and medical interventions can positively influence Klotho levels:

  • Regular Physical Activity: Endurance training and moderate exercise have been associated with higher Klotho levels in studies.
  • Vitamin D: Adequate vitamin D intake supports Klotho expression.
  • Caloric Moderation and Healthy Diet: A Mediterranean diet and caloric restriction have been shown to increase Klotho levels.
  • Stress Reduction: Chronic stress lowers Klotho levels; relaxation techniques may help counteract this effect.
  • Pharmacological Approaches: Certain medications such as ACE inhibitors, statins, and SGLT2 inhibitors have been associated with elevated Klotho levels.

Research and Future Perspectives

Klotho is considered one of the most promising targets in aging research. Clinical studies are currently investigating the therapeutic use of recombinant Klotho protein – for example, in chronic kidney disease, neurodegenerative conditions, and cardiovascular disease. Gene therapy approaches aimed at increasing endogenous Klotho production are also under investigation.
