Ein Koma ist ein tiefer Bewusstseinsverlust, bei dem die betroffene Person nicht auf äußere Reize reagiert und weder wach noch ansprechbar ist.
Es handelt sich um einen medizinischen Notfall, der auf eine schwere Schädigung des Gehirns oder eine Störung der Gehirnfunktion zurückzuführen ist. Ursachen können Schädel-Hirn-Trauma, Schlaganfall, Sauerstoffmangel (Hypoxie), Vergiftungen, Infektionen oder Stoffwechselstörungen wie Diabetes sein.
Das Koma unterscheidet sich vom Schlafzustand, da selbst starke Reize wie Schmerz, Licht oder Geräusche keine Reaktion hervorrufen. Ärztlich wird der Bewusstseinsgrad häufig mithilfe der Glasgow Coma Scale beurteilt, die Augenöffnen, verbale und motorische Reaktionen bewertet. Je niedriger der Wert, desto schwerer ist das Koma. Abhängig von Dauer und Ursache kann es sich in ein Wachkoma (apallisches Syndrom) oder einen vegetativen Zustand überführen.
Die Behandlung richtet sich nach der zugrunde liegenden Ursache. Wichtig sind Stabilisierung der Vitalfunktionen, Überwachung in einer Intensivstation und ggf. neurochirurgische oder internistische Maßnahmen. In manchen Fällen ist das Koma reversibel, etwa bei Stoffwechselentgleisungen, in anderen führt es zu dauerhaften neurologischen Schäden. Frühzeitige Behandlung sind entscheidend für die Prognose und Rehabilitation.
