Kreatinin (Creatin A) ist ein Abbauprodukt des Muskelstoffwechsels, das beim Abbau von Kreatinphosphat entsteht – einer energiereichen Verbindung, die in Muskelzellen gespeichert wird.
Kreatinin wird kontinuierlich über das Blut zur Niere transportiert und dort ausschließlich über den Urin ausgeschieden, ohne rückresorbiert zu werden. Aufgrund dieser stabilen Ausscheidung gilt Kreatinin als wichtiger Laborwert zur Einschätzung der Nierenfunktion, insbesondere der glomerulären Filtrationsrate (GFR). Ein erhöhter Kreatininwert im Blut deutet auf eine eingeschränkte Nierenfunktion hin, kann aber auch durch hohen Muskelanteil, intensives Training oder bestimmte Medikamente beeinflusst werden. Umgekehrt können sehr niedrige Werte bei älteren Menschen oder starkem Muskelabbau auftreten. Da Kreatinin alleine nicht ausreicht, um die Nierenleistung präzise zu bestimmen, wird heute häufig die eGFR (geschätzte glomeruläre Filtrationsrate) aus Kreatinin, Alter, Geschlecht und Körpergröße berechnet. Kreatinin wird auch im Urin gemessen, z. B. bei 24-Stunden-Sammelurin oder in der Verhältnisbestimmung zu Albumin (Albumin-Kreatinin-Quotient) zur Frühdiagnose von Nierenerkrankungen, z. B. bei Diabetes oder Bluthochdruck. Die regelmäßige Überprüfung von Kreatininwerten ist in der internistischen Diagnostik, Nephrologie und Sportmedizin essenziell.
Normwerte (können leicht laborabhängig variieren):
Kreatinin im Serum:
- Männer: 0,7–1,2 mg/dl
- Frauen: 0,5–1,0 mg/dl
eGFR:
- > 90 ml/min/1,73 m² = normal
- 60–89 = leicht eingeschränkt
- < 60 = pathologisch (chronische Niereninsuffizienz wahrscheinlich)
Kreatinin im Urin (24h-Sammelurin):
- Männer: 14–26 mg/kg/Tag
- Frauen: 11–20 mg/kg/Tag
