What is LAIV?

LAIV stands for Live Attenuated Influenza Vaccine. It is a type of influenza (flu) vaccine that contains live but weakened (attenuated) influenza viruses. Unlike traditional injectable flu vaccines, LAIV is administered as a nasal spray and is approved in many countries primarily for children and adolescents.

Mechanism of Action

The attenuated viruses in LAIV replicate in the cooler environment of the nasal mucosa but are unable to cause serious disease because they cannot grow effectively at the higher temperatures found deeper in the respiratory tract. This mimics a natural infection and stimulates the immune system on multiple levels:

  • Production of secretory IgA antibodies in the nasal lining (local mucosal immunity)
  • Activation of cellular immune responses (T-cells)
  • Generation of systemic antibodies in the bloodstream

This broad immune response may provide more comprehensive protection compared to purely injectable inactivated vaccines.

Composition and Variants

Like all flu vaccines, LAIV is updated annually to match the influenza strains recommended by the World Health Organization (WHO). The vaccine typically contains four strains (quadrivalent formulation): two influenza A strains and two influenza B strains. In Europe, LAIV is marketed under the brand name Fluenz Tetra.

Administration and Approval

LAIV is administered as a nasal spray – one dose into each nostril. Key administration details include:

  • Approved age group: In the EU, approved for children and adolescents aged 2 to 17 years
  • Dosage: Children under 9 years who have never been vaccinated against flu receive two doses at least 4 weeks apart; all others receive a single dose
  • Timing: Ideally administered before the start of the flu season, typically in autumn

  • Needle-free administration – especially beneficial for children with needle phobia
  • Broader and potentially stronger immune response by mimicking a natural infection
  • Good tolerability in healthy children

Side Effects

LAIV is generally well tolerated. Common side effects are mild and temporary:

  • Blocked or runny nose
  • Mild fever
  • Headache
  • Reduced appetite

Serious side effects are very rare. Because the vaccine contains live viruses, there is a theoretical risk of vaccine virus transmission to severely immunocompromised individuals in close contact.

Contraindications

LAIV should not be given to individuals with:

  • Severe immunosuppression (e.g., HIV infection, chemotherapy, congenital immune deficiencies)
  • Severe asthma or active wheezing
  • Known allergy to vaccine components (e.g., gelatin or gentamicin)
  • Use of antiviral medications against influenza (e.g., oseltamivir) within 48 hours before vaccination
  • Pregnancy
