What Are Langerhans Cells?

Langerhans cells are specialized dendritic cells found predominantly in the epidermis (outer layer of the skin). They were first described in 1868 by German medical student Paul Langerhans, after whom they are named. As part of both the innate and adaptive immune system, Langerhans cells play a crucial role in recognizing, processing, and presenting antigens (foreign substances) to other immune cells, thereby initiating targeted immune responses.

Structure and Characteristics

Langerhans cells are uniquely identified by their characteristic cytoplasmic organelles known as Birbeck granules, which are found exclusively in these cells and are involved in antigen uptake. On their cell surface, they express key markers including CD1a, CD207 (Langerin), and MHC class II molecules, all of which are essential for antigen recognition and presentation to T lymphocytes.

Function in the Immune System

The primary role of Langerhans cells is immune surveillance of the skin. Their main functions include:

  • Antigen uptake: Langerhans cells capture pathogens, foreign substances, and allergens through phagocytosis or endocytosis.
  • Antigen processing: Captured foreign material is broken down into small peptide fragments inside the cell.
  • Antigen presentation: Processed antigens are displayed on the cell surface via MHC molecules to activate T lymphocytes.
  • Migration: After encountering an antigen, Langerhans cells migrate through lymphatic vessels to regional lymph nodes, where they activate naive T cells and initiate adaptive immune responses.
  • Immune tolerance: Under certain conditions, Langerhans cells can also contribute to immune tolerance, helping to prevent excessive or harmful immune reactions.

Clinical Relevance

Langerhans cells are involved in several clinically important processes:

Allergic Reactions and Contact Dermatitis

Langerhans cells play a central role in allergic contact dermatitis. They capture contact allergens such as nickel or fragrances, migrate to lymph nodes, and sensitize T lymphocytes. Upon re-exposure to the allergen, these sensitized T cells trigger the typical inflammatory skin reaction.

Viral Infections

Langerhans cells are involved in defending the skin against viral infections, including those caused by the Human Papillomavirus (HPV) and Herpes simplex virus. Paradoxically, they can also serve as a route of entry for certain viruses such as HIV, potentially transporting them to deeper tissue layers and lymph nodes.

Langerhans Cell Histiocytosis

Langerhans cell histiocytosis (LCH) is a rare disorder characterized by the uncontrolled proliferation of Langerhans cells, which can infiltrate various organs including bone, lung, skin, liver, and brain. The exact cause is not fully understood, but BRAF-V600E mutations have been identified in many cases. The condition can occur at any age but is more common in children.

Tumor Immunology

Langerhans cells are being actively studied in the context of tumor immunology, as they may play a role in the recognition and elimination of skin cancer cells. Their precise function in anti-tumor immunity remains an active area of research.

Diagnosis

Langerhans cells are typically identified through immunohistochemical staining of tissue biopsy samples, using markers such as CD1a and Langerin (CD207). Electron microscopy can reveal the characteristic Birbeck granules. In cases of suspected Langerhans cell histiocytosis, molecular analyses are performed to detect genetic mutations such as the BRAF-V600E mutation.
