What is Lateral Malleolus Osteosynthesis?

Lateral malleolus osteosynthesis is a surgical procedure in trauma surgery and orthopedics in which a fractured lateral malleolus (the bony prominence on the outer side of the ankle) is stabilized using implants such as screws, plates, or wires. The primary goal is to restore the anatomical alignment of the ankle joint, ensuring proper joint function and long-term stability.

Causes and Indications

Lateral malleolus osteosynthesis is indicated in the following injuries:

  • Lateral malleolus fracture: A break of the outer ankle bone, commonly caused by a twisting injury (supination trauma)
  • Bimalleolar fracture: Simultaneous fracture of both the lateral and medial malleolus
  • Trimalleolar fracture: Additional fracture of the posterior tibial margin
  • Unstable ankle fractures according to the Weber classification (Type B and C)
  • Fractures involving the joint surface or displaced fragments that cannot be managed conservatively

Diagnosis

Diagnosis is typically established through:

  • Clinical examination: Swelling, pain, and tenderness over the lateral malleolus
  • X-rays of the ankle joint in two planes (anteroposterior and lateral), and occasionally stress views to assess ligament stability
  • Computed tomography (CT): For complex fractures to evaluate fragment displacement and joint involvement
  • MRI: Occasionally used to assess associated ligament injuries

Surgical Procedure

Preparation and Positioning

The patient is positioned in a supine or lateral decubitus position on the operating table. After application of a tourniquet and sterile draping, a skin incision is made laterally over the fibula.

Implants and Techniques

Depending on the fracture type and classification, different implants may be used:

  • One-third tubular plate or anatomical locking plate: Standard implant for Weber B and Weber C fractures, fixed laterally along the fibula
  • Lag screws: Used alone for simple oblique fractures or in combination with plate fixation
  • Intramedullary nail: An alternative technique for specific fracture patterns, particularly in elderly patients with osteoporosis
  • Syndesmotic screw or suture-button device: Used when there is an associated syndesmotic injury to stabilize the distal tibiofibular joint

Postoperative Care

After surgery, the ankle is initially immobilized in a splint or plaster cast. Weight-bearing is gradually increased according to the surgeon's protocol. Physiotherapy and range-of-motion exercises are initiated as part of the rehabilitation process.

Risks and Complications

As with any surgical procedure, lateral malleolus osteosynthesis carries potential risks:

  • Wound healing complications and infection
  • Nerve or vascular injury in the surgical area
  • Implant failure or loosening
  • Nonunion (failure of bone healing)
  • Post-traumatic osteoarthritis of the ankle joint
  • Deep vein thrombosis or pulmonary embolism

Prognosis and Rehabilitation

With proper surgical fixation and consistent post-operative care, the prognosis for lateral malleolus fractures is generally favorable. Full weight-bearing is typically achieved within 6 to 12 weeks. Return to full sporting activity may take 3 to 6 months. Early and closely supervised physiotherapy is essential for achieving optimal functional outcomes.
