What is Lateral Pharyngitis?

Lateral pharyngitis (also called lateral band angina or lateral pharyngeal band inflammation) is an inflammation of the lateral pharyngeal bands -- narrow strips of lymphatic tissue running along the sides of the back of the throat. These bands are part of the immune system of the upper respiratory tract and can become inflamed when pathogens invade the area.

This condition is particularly common in individuals who have had their tonsils (palatine tonsils) surgically removed. In such cases, the lymphatic tissue of the lateral bands takes on a compensatory immune function and may become more susceptible to infection and inflammation.

Causes

Lateral pharyngitis is most commonly caused by infectious agents:

  • Bacterial pathogens: Group A streptococci (especially Streptococcus pyogenes) and staphylococci are frequent causes.
  • Viral pathogens: Rhinoviruses, adenoviruses, and the Epstein-Barr virus can also trigger inflammation of the lateral bands.
  • Predisposing factors: A weakened immune system, exposure to cold, smoking, mouth breathing, and dry air can increase the risk of developing this condition.

Symptoms

The typical symptoms of lateral pharyngitis include:

  • Severe, one-sided or bilateral sore throat
  • Difficulty swallowing (odynophagia)
  • Pain radiating to one or both ears
  • Fever, often exceeding 38.5 degrees Celsius
  • General feeling of illness, fatigue, and malaise
  • Swollen lymph nodes in the neck
  • Reddened, swollen lateral bands in the throat, sometimes with yellowish-white deposits

Diagnosis

Diagnosis is typically made through a clinical examination. The physician inspects the throat and assesses the redness and swelling of the lateral bands. To distinguish between bacterial and viral infections, the following measures may be used:

  • Throat swab: To identify the causative pathogen, especially to detect Group A streptococci.
  • Rapid streptococcal antigen test: Provides results within a few minutes and can help determine whether antibiotic therapy is necessary.
  • Blood count: May be performed if a more complex infection or systemic involvement is suspected.

Treatment

General Measures

Regardless of the underlying cause, the following supportive measures are generally recommended:

  • Adequate rest and physical recovery
  • Drinking plenty of fluids such as water and herbal teas
  • Gargling with salt water or antiseptic solutions to relieve symptoms
  • Throat lozenges with local anesthetic or antiseptic properties
  • Warm neck compresses to ease pain and discomfort

Medical Treatment

  • Pain relievers and fever reducers: Ibuprofen or paracetamol (acetaminophen) can effectively relieve pain and reduce fever.
  • Antibiotics: When a bacterial infection has been confirmed -- particularly by streptococci -- antibiotic therapy is initiated, most commonly with penicillin or amoxicillin. The full course must be completed as prescribed (typically 7--10 days) to prevent complications and antibiotic resistance.

Complications

In rare cases, untreated or poorly managed lateral pharyngitis can lead to complications, including:

  • Peritonsillar or parapharyngeal abscess
  • Spread of infection to adjacent structures
  • In streptococcal infections: rheumatic fever or kidney disease (post-streptococcal glomerulonephritis)
