What Is Ligament Augmentation?

Ligament augmentation is a surgical technique in which a damaged or torn ligament is mechanically reinforced or supported by additional material. The primary goal is to restore joint stability while preserving the natural healing capacity of the existing ligament tissue. Unlike full ligament reconstruction, where the ligament is entirely replaced, augmentation aims to retain as much of the original tissue as possible.

Indications

Ligament augmentation is most commonly used in the following clinical settings:

  • Knee joint: Most frequently for tears of the anterior cruciate ligament (ACL), as well as posterior cruciate ligament and collateral ligament injuries.
  • Ankle joint: For chronic lateral ankle instability following recurrent sprains.
  • Shoulder and elbow: Less commonly, in cases of ligamentous instability of these joints.

Mechanism and Techniques

In ligament augmentation, the damaged ligament is supported by an additional material acting as an internal reinforcement. Several approaches exist:

Synthetic Augmentation

A synthetic implant – for example, made of polyethylene fibers or other biocompatible materials – is used as an internal brace. This immediately stabilizes the joint and protects the healing ligament from excessive stress during the recovery period.

Biological Augmentation

Biological materials such as autografts (the patient's own tendons) or allografts (donor tendons) can be used as supplementary reinforcement. This approach promotes biological integration and may support long-term structural integrity.

Dynamic Augmentation

Innovative techniques such as the BEAR implant (Bridge-Enhanced ACL Restoration) combine biological factors (such as the patient's own blood) with a collagen scaffold to actively stimulate natural ligament healing.

Risks and Possible Complications

As with any surgical procedure, ligament augmentation carries certain risks:

  • Infection at the surgical site
  • Foreign body reactions to synthetic materials
  • Failure of the augmentation material (re-rupture)
  • Joint effusion or persistent swelling
  • Residual instability if healing is incomplete

Aftercare and Rehabilitation

Structured physiotherapy is essential following ligament augmentation. A typical rehabilitation program includes:

  • Controlled weight-bearing with gradual load progression in the initial weeks
  • Exercises to restore muscle strength, balance, and coordination
  • Sport-specific training in later rehabilitation phases
  • Regular follow-up appointments to monitor healing progress

The total rehabilitation period varies depending on the joint involved, the severity of the injury, and the technique used, but generally spans several months.
