What Are Lipid Metabolism Markers?

Lipid metabolism markers are laboratory blood test values that provide information about how the body processes fats (lipids). They help physicians assess the risk of cardiovascular diseases such as heart attack or stroke, and to identify conditions such as dyslipidemia (a disorder of lipid metabolism). Regular measurement of these markers is an important component of preventive health screenings.

Key Lipid Metabolism Markers

Total Cholesterol

Total cholesterol is the sum of all cholesterol fractions in the blood. Elevated levels may indicate an increased risk of heart disease, but should always be evaluated in conjunction with the individual cholesterol fractions. According to European guidelines, the ideal level for adults is below 200 mg/dl (5.2 mmol/l).

LDL Cholesterol

LDL cholesterol (low-density lipoprotein cholesterol) is commonly referred to as bad cholesterol because elevated levels contribute to the buildup of fatty plaques in the artery walls (atherosclerosis). Target values vary depending on the individual risk profile of the patient.

HDL Cholesterol

HDL cholesterol (high-density lipoprotein cholesterol) is often called good cholesterol because it transports excess cholesterol from the blood vessels back to the liver for breakdown. Low HDL levels are considered a risk factor for cardiovascular disease. Desirable levels are above 40 mg/dl in men and above 50 mg/dl in women.

Triglycerides

Triglycerides are the most common form of dietary fat transported in the blood. Elevated triglyceride levels, especially in combination with low HDL cholesterol, increase the risk of cardiovascular disease and pancreatitis (inflammation of the pancreas). The normal value is below 150 mg/dl.

Non-HDL Cholesterol

Non-HDL cholesterol is calculated by subtracting HDL cholesterol from total cholesterol. It encompasses all atherogenic (vessel-damaging) lipoproteins and is considered a more comprehensive risk marker, particularly when triglyceride levels are elevated.

Lipoprotein(a)

Lipoprotein(a), abbreviated as Lp(a), is a genetically determined independent risk factor for atherosclerosis and heart disease. Unlike other lipid markers, Lp(a) levels can hardly be influenced by lifestyle changes.

Apolipoproteins

Apolipoprotein B (ApoB) and Apolipoprotein A-I (ApoA-I) are protein components of lipoproteins. ApoB reflects the total burden of atherogenic particles, while ApoA-I is associated with HDL cholesterol. Both serve as complementary risk markers in lipid assessment.

When Are Lipid Metabolism Markers Measured?

Testing lipid metabolism markers is recommended in the following situations:

  • Routine preventive health screenings from age 35 onwards
  • Known family history of lipid metabolism disorders
  • Overweight or obesity
  • Diabetes mellitus or insulin resistance
  • High blood pressure (arterial hypertension)
  • Suspected or confirmed cardiovascular disease
  • Monitoring of ongoing therapy (e.g., with statins)

Diagnosis and Blood Test Procedure

Lipid metabolism markers are typically measured from a venous blood sample. Since triglycerides in particular fluctuate significantly depending on recent food intake, a fasting period of at least 8 to 12 hours before blood collection is recommended. Values are reported in mg/dl or mmol/l and assessed in the context of individual risk factors.

Treatment for Abnormal Lipid Metabolism Markers

Lifestyle Changes

For mildly elevated values, non-pharmacological measures are the first line of action:

  • Dietary adjustments: reducing saturated fats and trans fats, increasing fiber and unsaturated fatty acids
  • Regular physical activity (at least 150 minutes of moderate exercise per week)
  • Weight loss in cases of overweight
  • Smoking cessation and reduction of alcohol consumption

Medical Treatment

When lifestyle changes are insufficient, lipid-lowering medications may be prescribed:

  • Statins (e.g., atorvastatin, simvastatin): inhibit the liver's own cholesterol synthesis
  • Ezetimibe: reduces cholesterol absorption in the intestine
  • PCSK9 inhibitors: antibodies that promote LDL cholesterol breakdown in the liver
  • Fibrates: primarily lower elevated triglyceride levels
