What is Liver Cell Therapy?

Liver cell therapy, also known as hepatocyte transplantation, is a medical treatment in which isolated, functional liver cells – called hepatocytes – are introduced into a patient's body. The goal is to support or partially replace a damaged or dysfunctional liver. It is considered a promising alternative or complement to full liver organ transplantation.

How Does Liver Cell Therapy Work?

In liver cell therapy, hepatocytes are harvested from a healthy donor liver, processed, and then infused into the recipient. The most common route of administration is through the portal vein, which delivers the cells directly to the liver. Once there, the transplanted cells engraft into the liver tissue and begin to take over hepatic functions.

Types of Liver Cell Therapy

  • Allogeneic hepatocyte transplantation: Liver cells are obtained from a human donor.
  • Xenogeneic cell therapy: Use of animal-derived liver cells, such as from pigs, primarily in research settings.
  • Stem cell-based approaches: Use of hepatocytes derived from stem cells (currently under clinical investigation).

Indications and Applications

Liver cell therapy is used or investigated for a range of liver conditions:

  • Acute liver failure: As a bridge therapy until organ transplantation or spontaneous liver recovery.
  • Inherited metabolic liver diseases: Such as Crigler-Najjar syndrome (bilirubin metabolism disorder) or urea cycle disorders.
  • Chronic liver disease: As a supportive measure in advanced liver cirrhosis.

Advantages Over Full Liver Transplantation

Compared to whole organ transplantation, liver cell therapy offers several potential advantages:

  • Less invasive – no major surgery required.
  • Cells can be administered in repeated doses.
  • A single donor liver can be used to treat multiple patients.
  • The patient's own liver is preserved in case regeneration is possible.

Risks and Challenges

Like all medical interventions, liver cell therapy carries certain risks and limitations:

  • Immune rejection: The recipient's immune system may attack the transplanted cells, requiring immunosuppressive therapy.
  • Limited cell availability: Suitable donor hepatocytes are in limited supply.
  • Reduced long-term efficacy: Transplanted cells may gradually lose their functional capacity over time.
  • Still experimental: Many applications remain in clinical trial phases and are not yet widely available.

Current State of Research

Liver cell therapy is an active and rapidly evolving field of research. Scientists are working on improving cell isolation and preservation techniques, exploring new hepatocyte sources such as induced pluripotent stem cells (iPSCs), and developing better immunosuppression protocols. Early clinical studies have shown encouraging results, particularly in children with inherited metabolic disorders.
