Methods of Magnesium Balance Monitoring
Serum Magnesium Measurement
The most commonly used diagnostic parameter is the serum magnesium level. In adults, the normal range is generally between 0.75 and 1.05 mmol/L. However, this value provides limited insight into total body magnesium status, as only about 1% of total body magnesium is found in the blood.
Urinary Magnesium Excretion
Measuring magnesium in a 24-hour urine collection provides a more accurate assessment of renal regulation. Low excretion may indicate deficiency, while elevated levels may suggest renal losses or excess intake.
Magnesium Loading Test
In the magnesium retention test, a defined amount of magnesium is administered intravenously, and subsequent urinary excretion is measured. If more than 20% of the administered magnesium is retained by the body, this is considered indicative of intracellular magnesium deficiency.
Ionized Magnesium
In specialized laboratories, ionized (free) magnesium in the blood can be measured, representing the biologically active form. This measurement is more precise but more complex and not universally available.

Magnesium Deficiency (Hypomagnesemia)
A serum magnesium level below 0.75 mmol/L is referred to as hypomagnesemia. Symptoms may include:

Magnesium Excess (Hypermagnesemia)
