What is Malaria Prophylaxis?

Malaria prophylaxis refers to all preventive measures taken to reduce or eliminate the risk of contracting malaria. Malaria is a life-threatening infectious disease caused by Plasmodium parasites, transmitted through the bite of infected female Anopheles mosquitoes. Travelers visiting tropical and subtropical regions in sub-Saharan Africa, South Asia, Southeast Asia, and Latin America are particularly at risk.

Why is Malaria Prophylaxis Important?

Unlike residents of malaria-endemic regions who may develop partial immunity over time, travelers from non-endemic countries have no natural protection against the disease. Without appropriate preventive measures, infection with Plasmodium falciparum – the most dangerous malaria species – can become life-threatening within days. Early, individually tailored prophylaxis is therefore strongly recommended.

Types of Malaria Prophylaxis

1. Exposure Prophylaxis (Protection Against Mosquito Bites)

Preventing mosquito bites is the first and most fundamental protective measure, as Anopheles mosquitoes are primarily active at dusk and during the night:

Wearing long-sleeved, light-colored clothing during evening and night hours
Applying insect repellents (repellents) to skin and clothing, such as DEET or Icaridin
Sleeping under a mosquito net, ideally one that is insecticide-treated (LLIN – Long-Lasting Insecticidal Net)
Using air conditioning or window screens in accommodations

2. Medication-Based Prophylaxis (Chemoprophylaxis)

Depending on the travel destination and individual risk profile, travel medicine specialists may recommend antimalarial medication. The following active ingredients are commonly used:

Atovaquone/Proguanil (brand name e.g. Malarone): Well-tolerated, taken shortly before, during, and shortly after travel. Suitable for high-risk areas.
Doxycycline: A broad-spectrum antibiotic also used as malaria prophylaxis. Must be taken daily and increases skin sensitivity to sunlight.
Mefloquine (brand name e.g. Lariam): Taken weekly, but associated with potential neuropsychiatric side effects. Less commonly used today.
Chloroquine: Only used in a few regions where the parasite remains susceptible, due to widespread global resistance.

3. Stand-by Treatment (Emergency Self-Treatment)

For travelers to remote areas without quick access to medical care, a stand-by emergency treatment may be recommended. The traveler carries an antimalarial medication and initiates treatment independently if typical symptoms develop, until professional medical help can be reached.

Who Needs Malaria Prophylaxis?

Not every trip to a tropical country automatically requires medication-based prophylaxis. The decision depends on several factors:

Travel destination and risk classification of the area (e.g., according to WHO or national travel medicine guidelines)
Duration and style of travel (e.g., backpacking vs. city hotel)
Health status and pre-existing medical conditions of the traveler
Pregnancy or planned pregnancy
Concurrent medications (potential drug interactions)

An individual travel medicine consultation, ideally 4–6 weeks before departure, is strongly recommended.

Side Effects and Contraindications

All antimalarial medications can cause side effects. Pre-existing conditions, allergies, and current medications should be discussed with a physician before starting chemoprophylaxis. Special caution is advised for:

Pregnant and breastfeeding women (not all medications are suitable)
Children (dosage adjustment required)
Individuals with liver or kidney disease
People with epilepsy or psychiatric conditions (particularly regarding Mefloquine)
