What Is the Manifestation Index?
The manifestation index is an epidemiological measure that describes what proportion of all individuals infected with a pathogen actually develop a clinically recognizable illness. It is calculated as the ratio of manifest (symptomatic) cases to the total number of infected individuals, including those with inapparent (asymptomatic) infections.
A high manifestation index means that a large proportion of infected individuals become clinically ill. A low manifestation index, on the other hand, indicates that many infections occur without any recognizable symptoms – so-called subclinical or inapparent infections.
Calculation
The formula is as follows:
Manifestation Index = (Number of manifest cases) / (Total number of infected individuals) x 100%
The total number of infected individuals includes both symptomatic and asymptomatic cases. Since asymptomatic infections often go undetected, the manifestation index is frequently determined in practice through serological studies, in which specific antibodies serve as evidence that an infection has occurred.
Clinical and Epidemiological Significance
The manifestation index is a central tool in infectious disease epidemiology. It provides important information for:
- Assessing the true spread of an infectious disease within a population
- Planning vaccination campaigns and preventive measures
- Evaluating the severity and danger of a pathogen
- Modeling infectious diseases (e.g., for pandemic planning)
A low manifestation index can lead to a significant underestimation of the true number of infections in a population, since many affected individuals do not seek medical attention and therefore are not captured in official statistics.
Examples from Medicine
Different infectious diseases show very different manifestation indices:
- Poliovirus (Type 1): The manifestation index is very low. Only about 0.1–1% of infected individuals develop paralytic poliomyelitis; the majority of infections are inapparent.
- Measles virus: The manifestation index is very high (close to 100%), as nearly all infected individuals develop a typical clinical picture.
- Epstein-Barr virus (EBV): Primary infection in childhood is often asymptomatic, whereas infection in adolescents and adults more frequently manifests as infectious mononucleosis.
- SARS-CoV-2: The manifestation index varied considerably depending on the virus variant, vaccination status, and risk group, and was the subject of extensive epidemiological research.

The manifestation index should not be confused with the following related concepts:
- Case Fatality Rate (CFR): The proportion of diagnosed individuals who die from the disease.
- Mortality rate: The number of deaths from a disease relative to the total population.
