Marsupialisation (also spelled marsupialization) is a surgical technique used to treat cysts and abscesses. The term derives from the Latin word marsupium, meaning pouch, which reflects the principle of the procedure: the cyst wall is incised and sutured to the surrounding skin or mucous membrane to create a permanent opening. This opening allows the continuous drainage of fluid and prevents the cyst from resealing.
Indications and Areas of Use
Marsupialisation is performed across several medical specialties. The most common indications include:
- Bartholin gland cyst and abscess: The most frequent indication in gynaecology. The Bartholin gland, located near the vaginal opening, can become blocked and form a painful cyst or abscess.
- Ranula: A mucous retention cyst on the floor of the mouth caused by a blocked salivary gland.
- Pancreatic cysts: Used in abdominal surgery for selected cysts of the pancreas.
- Pilonidal cyst: A cyst near the coccyx (tailbone), most commonly seen in younger adults.
- Oral and jaw cysts: Applied in oral and maxillofacial surgery and dentistry for large jaw cysts.
How is Marsupialisation Performed?
The procedure is typically performed under local or regional anaesthesia, depending on the size and location of the cyst. Larger or deeper cysts may require general anaesthesia.
The procedure involves the following steps:
- The skin or mucous membrane overlying the cyst is disinfected and, if needed, anaesthetised locally.
- An elliptical or oval incision is made into the cyst wall, removing a portion of it.
- The cyst contents (fluid, pus, or secretion) are fully drained.
- The edges of the remaining cyst wall are sutured to the surrounding skin or mucosa, forming a permanently open pouch.
- The resulting cavity heals by granulation from the inside outward, gradually reducing in size.
Advantages Over Other Procedures
Compared to complete surgical removal (excision) of a cyst, marsupialisation offers several advantages:
- Lower risk of injury to surrounding structures (e.g., glandular tissue, nerve pathways)
- Shorter operating time
- Reduced risk of bleeding
- Preservation of gland function (e.g., the Bartholin gland)
Aftercare and Healing
Careful wound care after marsupialisation is essential to prevent infection and promote healing. Regular sitz baths or irrigations are often recommended. Healing typically occurs within a few weeks. The recurrence rate is low when the procedure is performed correctly, though recurrence is not entirely excluded.

As with any surgical procedure, marsupialisation carries potential risks, including:
- Post-operative bleeding
- Wound infection
- Recurrence of the cyst
- Scar formation
- Rarely: injury to adjacent structures
