Personal details: Name, date of birth, address, and contact information of the expectant mother.
Medical history: Pre-existing conditions, previous pregnancies and births, allergies, and family medical history.
Laboratory results: Blood type, Rhesus factor, antibody screening, rubella immunity status, infectious disease screening (e.g., syphilis, HIV, hepatitis B), blood glucose, and urinalysis results.
Pregnancy progress: Results of regular check-up appointments, blood pressure measurements, weight development, and fetal presentation.
Ultrasound findings: Results of the three standard ultrasound examinations (1st, 2nd, and 3rd trimester), including fetal measurements and position.

Importance of Prenatal Check-ups
