What Is the MCH Value?

The MCH value (Mean Corpuscular Hemoglobin) is a laboratory parameter included in a complete blood count (CBC). It represents the average amount of hemoglobin – the red pigment responsible for oxygen transport – contained within a single red blood cell (erythrocyte).

The MCH value is expressed in picograms (pg) and is calculated by dividing the total hemoglobin content of the blood by the number of red blood cells.

Normal Range

In adults, the normal MCH range is generally between 28 and 34 pg. Values outside this range may indicate an underlying medical condition or nutritional deficiency. Reference ranges may vary slightly between laboratories.

Elevated MCH Value

An MCH value above 34 pg is called hyperchromia, meaning each red blood cell contains more hemoglobin than usual. Common causes include:

  • Vitamin B12 deficiency: Leads to megaloblastic anemia, where red blood cells are abnormally large and hemoglobin-rich.
  • Folate deficiency: Causes a similar type of anemia as vitamin B12 deficiency.
  • Liver disease: Can affect red blood cell production and maturation.
  • Alcohol abuse: Interferes with the normal development of erythrocytes.
  • Hypothyroidism: An underactive thyroid can impair blood cell formation.

Low MCH Value

An MCH value below 28 pg is referred to as hypochromia, indicating that red blood cells contain less hemoglobin than normal. Typical causes include:

  • Iron deficiency anemia: The most common cause worldwide. Without sufficient iron, hemoglobin synthesis is impaired.
  • Thalassemia: An inherited disorder in which hemoglobin production is genetically disrupted.
  • Chronic inflammatory diseases: Can limit the body's ability to use iron effectively.
  • Sideroblastic anemia: A rare disorder of hemoglobin synthesis despite adequate iron stores.

Diagnosis and Clinical Relevance

The MCH value is always interpreted alongside other blood count parameters, particularly:

  • MCV (Mean Corpuscular Volume): The average size of a red blood cell.
  • MCHC (Mean Corpuscular Hemoglobin Concentration): The average concentration of hemoglobin within red blood cells.
  • Hemoglobin and hematocrit: Additional key parameters for assessing overall blood health.

By combining these values, healthcare providers can determine the type of anemia more precisely and identify its underlying cause. For example, a low MCH combined with a low MCV is characteristic of iron deficiency anemia, while an elevated MCH alongside an elevated MCV suggests vitamin B12 or folate deficiency.

When to See a Doctor

Abnormal MCH values are typically discovered during routine blood tests. Symptoms that may indicate anemia and warrant medical evaluation include:

  • Persistent fatigue and exhaustion
  • Paleness of the skin or mucous membranes
  • Shortness of breath during physical activity
  • Rapid heartbeat or dizziness
  • Difficulty concentrating

If an abnormal MCH value or anemia is suspected, medical advice should always be sought to identify the cause and initiate appropriate treatment.
