Medicinal leeches (Hirudo medicinalis) are parasitic annelid worms belonging to the class Clitellata. They inhabit freshwater environments and feed on the blood of vertebrates. In medicine, specially bred, sterile leeches are used for therapeutic purposes. Leech therapy, also known as hirudotherapy, is one of the oldest healing methods known to humanity and is today experiencing a scientifically grounded revival.
The medical use of leeches dates back to ancient times. In ancient Egypt, Greece, and Rome, they were used to treat fever, inflammation, and venous congestion. In the 19th century, leech therapy reached its peak popularity in Europe before being largely replaced by modern medicine. Today, it is recognized as a complementary therapy and is being investigated scientifically.
During the sucking process, the leech releases a variety of bioactive substances from its salivary glands into the wound. The most important active compounds include:
The combination of blood removal, anticoagulant, and anti-inflammatory substances creates an overall therapeutic effect that can relieve local circulatory disorders, swelling, and pain.
Leech therapy is used today in various medical fields:
Leech therapy is performed exclusively by trained medical professionals. Only approved, sterile medicinal leeches are used, and they are professionally disposed of after a single use to eliminate any risk of infection transmission. Depending on the indication, between 2 and 10 leeches are applied to the affected area of the body. A single leech draws approximately 5 to 15 ml of blood over 30 to 60 minutes. After the leech detaches, the wound continues to bleed for several hours due to the anticoagulant substances, which is therapeutically intended.
Leech therapy is considered safe when properly administered. Possible side effects include:
Contraindications include bleeding disorders, use of anticoagulant medications, anemia, immunodeficiency, and pregnancy.
The effectiveness of leech therapy is scientifically supported for certain indications. The use for knee osteoarthritis is particularly well-studied, with several clinical trials demonstrating significant pain reduction and improved joint function. In microsurgery, the application is now an established standard. Further research is needed for other areas of application.
