Metformin ist ein orales Antidiabetikum und gilt als Standardtherapie bei Typ-2-Diabetes.
Metformin wird seit Jahrzehnten erfolgreich eingesetzt, um den Blutzuckerspiegel zu senken und die Insulinempfindlichkeit der Zellen zu verbessern. Im Gegensatz zu Insulin oder anderen Wirkstoffen fördert Metformin nicht die Insulinausschüttung, sondern wirkt vor allem durch Hemmung der Glukoseproduktion in der Leber (Hepatoglukoneogenese) und Verbesserung der Glukoseaufnahme in die Muskulatur.
Die Wirkung von Metformin setzt nicht sofort, sondern nach einigen Tagen regelmäßiger Einnahme ein. Es führt typischerweise zu keiner Gewichtszunahme. Auch das Risiko für kardiovaskuläre Komplikationen kann durch Metformin langfristig reduziert werden. Es gilt daher als besonders wertvoll für übergewichtige Diabetiker mit metabolischem Syndrom.
Trotz seiner Vorteile kann Metformin Nebenwirkungen verursachen, insbesondere gastrointestinale Beschwerden wie Blähungen, Übelkeit oder Durchfall. Eine seltene, aber ernsthafte Komplikation ist die Laktatazidose, weshalb Metformin bei bestimmten Vorerkrankungen (z. B. Niereninsuffizienz) kontraindiziert ist. Auch in der Prävention bei Prädiabetes und bei PCOS (polyzystisches Ovarialsyndrom) wird es off-label eingesetzt.
Wirkmechanismus von Metformin:
- Hemmung der Glukoseneubildung in der Leber
- Verbesserte Insulinwirkung an Muskel- und Fettzellen
- Verzögerte Glukoseaufnahme im Darm
- Keine direkte Stimulation der Bauchspeicheldrüse
Anwendungsgebiete:
- Typ-2-Diabetes mellitus (Standardtherapie)
- Prädiabetes, metabolisches Syndrom
- PCOS (off-label)
- In Kombination mit anderen Antidiabetika oder Insulin
Häufige Nebenwirkungen:
- Magen-Darm-Beschwerden: Übelkeit, Blähungen, Durchfall
- Geschmacksstörungen
- In seltenen Fällen: Laktatazidose (lebensbedrohlich!)
- Langfristig: Risiko für Vitamin-B12-Mangel
