What Is the Milk Ejection Reflex?
The milk ejection reflex (also known as the let-down reflex) is a physiological process that occurs during breastfeeding. It actively propels breast milk from the milk-producing glands toward the nipple, making it available for the baby to drink. Without this reflex, a baby would be unable to obtain milk effectively, even if the mother is producing an adequate supply. It is one of the most important mechanisms for successful breastfeeding.
Mechanism of Action
The milk ejection reflex is triggered by the hormone oxytocin, which is produced in the hypothalamus and released into the bloodstream by the pituitary gland. The process unfolds as follows:
- The baby suckling at the nipple sends nerve signals to the brain.
- The brain responds by releasing oxytocin into the bloodstream.
- Oxytocin causes the myoepithelial cells (smooth muscle cells surrounding the milk alveoli) to contract.
- This contraction pushes milk out of the alveoli, through the milk ducts, and toward the nipple.
The reflex can also be triggered by other stimuli, such as hearing the baby cry, thinking about the baby, or skin-to-skin contact.
How Does the Let-Down Feel?
Many breastfeeding mothers can perceive the milk ejection reflex. Common sensations include:
- A tingling or pulling feeling in the breast
- A sense of fullness or pressure in the breast
- Milk leaking from the opposite, non-nursing breast
- A change in the baby to deeper, rhythmic swallowing
Some mothers feel little or no sensation, even when the reflex is functioning normally. The absence of a noticeable let-down does not mean that milk is not flowing.
Factors That Can Inhibit the Reflex
The milk ejection reflex can be suppressed or weakened by several factors:
- Stress and anxiety: The stress hormone adrenaline inhibits oxytocin release and can interfere with the reflex.
- Pain: Significant pain during breastfeeding can block the reflex.
- Fatigue and exhaustion: Chronic tiredness negatively affects the hormonal balance.
- Poor latch: An inadequate latch reduces stimulation of the nipple and areola.
Supportive measures such as relaxation techniques, a calm breastfeeding environment, warmth applied to the breast, and professional lactation support can help improve the reflex.
Importance for Breastfeeding
The milk ejection reflex is essential for adequate milk delivery. Together with milk production (regulated by the hormone prolactin), it forms the second key hormonal mechanism of breastfeeding. A well-functioning reflex ensures that the baby receives sufficient milk, that the breast is emptied regularly, and that milk production is maintained.
