Mirtazapine is an antidepressant used to treat depression. It works by influencing neurotransmitters in the brain, helping to improve mood, sleep, and appetite.

What is Mirtazapine?

Mirtazapine is a prescription medication belonging to the class of noradrenergic and specific serotonergic antidepressants (NaSSA). It is primarily used to treat depression and is available under brand names such as Remeron, as well as various generic formulations.

Indications

Mirtazapine is mainly prescribed for moderate to severe depression. Common indications include:

  • Unipolar depression, especially when accompanied by anxiety and sleep disturbances
  • Depression with loss of appetite and weight loss
  • Depression in elderly patients
  • Adjunct therapy for insomnia associated with depression

Mechanism of Action

Mirtazapine works differently from classical antidepressants such as SSRIs (selective serotonin reuptake inhibitors). It blocks specific receptors in the brain:

  • Alpha-2 adrenoreceptors: Blocking these receptors increases the release of both noradrenaline and serotonin.
  • 5-HT2 and 5-HT3 serotonin receptors: Selective blockade of these receptors enhances the antidepressant effect while reducing side effects such as nausea or sexual dysfunction.
  • H1 histamine receptors: Blockade of these receptors accounts for the sedating and sleep-promoting properties of mirtazapine, particularly at lower doses.

Dosage and Administration

Mirtazapine is typically taken once daily, preferably in the evening before bedtime. The usual starting dose is 15 mg per day, which may be gradually increased to 45 mg per day depending on response and tolerability, always under medical supervision. An orally disintegrating tablet formulation is also available.

The full antidepressant effect usually becomes apparent after 2 to 4 weeks of regular use. The medication should not be stopped abruptly but should be tapered gradually under medical guidance.

Side Effects

Like all medications, mirtazapine can cause side effects. Common side effects include:

  • Drowsiness and sedation (particularly at the start of treatment)
  • Weight gain and increased appetite
  • Dry mouth
  • Dizziness
  • Constipation

Less common side effects may include:

  • Restlessness (akathisia)
  • Elevated liver enzymes
  • Reduction in white blood cell count (agranulocytosis) -- very rare but serious
  • Hyponatraemia (low blood sodium levels)

Interactions

Mirtazapine can interact with other medications. The most clinically significant interactions include:

  • MAO inhibitors: Concurrent use is contraindicated due to the risk of serotonin syndrome.
  • Other antidepressants (e.g., SSRIs, SNRIs): Increased risk of serotonin syndrome.
  • Benzodiazepines and other sedatives: Enhanced sedation may occur.
  • Alcohol: Should be avoided during treatment.

Precautions and Warnings

Mirtazapine should be used with caution in patients with:

  • A history of epilepsy or seizures
  • Impaired kidney or liver function
  • Cardiovascular disease
  • Diabetes mellitus
  • Narrow-angle glaucoma

Use in children and adolescents under 18 years of age is generally not approved. Pregnant or breastfeeding women should only take mirtazapine after a careful benefit-risk assessment with their healthcare provider.
