Mucosal healing refers to the restoration of the intestinal lining in inflammatory bowel disease. It is a key treatment goal in Crohn's disease and ulcerative colitis.

What Is Mucosal Healing?

Mucosal healing describes the complete or near-complete restoration of the intestinal mucosa (the inner lining of the gut) following inflammatory damage. In chronic inflammatory bowel diseases (IBD) such as Crohn's disease and ulcerative colitis, the lining of the gastrointestinal tract is damaged by persistent inflammation. Mucosal healing means that this damage resolves and the mucosa regains its normal structure and protective function.

Clinical Importance

Mucosal healing has emerged as one of the most important treatment targets in IBD management. In the past, clinical remission -- meaning the absence of symptoms -- was considered a sufficient goal. It is now understood that being symptom-free does not always mean the mucosa has fully healed.

  • Patients with confirmed mucosal healing have a significantly lower risk of disease relapse.
  • The risk of complications such as intestinal strictures, fistulas, or the need for surgery is substantially reduced.
  • The long-term risk of colorectal cancer in ulcerative colitis is decreased.
  • Overall quality of life improves sustainably for affected individuals.

Diagnosis and Assessment

Mucosal healing is primarily assessed through endoscopy (colonoscopy or sigmoidoscopy), which allows direct visualization of the intestinal lining. Histological examination of biopsy samples can provide evidence of microscopic healing. In addition, biomarkers in blood and stool, such as fecal calprotectin and C-reactive protein (CRP), can indicate the degree of ongoing inflammation.

Endoscopic Scoring Systems

Validated scoring systems are used for standardized assessment of mucosal healing, including:

  • Mayo Endoscopic Score for ulcerative colitis
  • Simple Endoscopic Score for Crohn's Disease (SES-CD)
  • Rutgeerts Score for postoperative recurrence assessment in Crohn's disease

Treatment to Achieve Mucosal Healing

Several drug therapies aim to induce and maintain mucosal healing:

Biologics

Biologics are biotechnologically produced agents that specifically target inflammatory pathways. They are currently the most effective medications for achieving mucosal healing. Key groups include:

  • TNF-alpha inhibitors (e.g., infliximab, adalimumab)
  • Integrin antagonists (e.g., vedolizumab)
  • IL-12/23 inhibitors (e.g., ustekinumab)
  • IL-23 inhibitors (e.g., risankizumab)

Small Molecules

Small molecules are chemically synthesized compounds that also inhibit inflammatory pathways, such as JAK inhibitors (e.g., tofacitinib, upadacitinib).

Other Medications

Aminosalicylates (e.g., mesalazine) in ulcerative colitis and immunomodulators (e.g., azathioprine) can also contribute to mucosal healing, although they are generally less potent than biologics.

Histological Healing as an Extended Goal

Beyond endoscopically visible mucosal healing, histological healing is gaining increasing attention. It describes the complete normalization of tissue at the microscopic level, meaning the absence of any inflammatory cell infiltrates. Studies indicate that histological healing is associated with even better long-term outcomes than endoscopic remission alone.
