What is Myditin?

Myditin is a prescription medication containing the active ingredient pyridostigmine bromide. It belongs to the pharmacological class of cholinesterase inhibitors and is primarily used to treat myasthenia gravis, a chronic autoimmune neuromuscular disorder. In myasthenia gravis, the body's immune system attacks the acetylcholine receptors at the neuromuscular junction, impairing the transmission of nerve impulses to muscles and resulting in abnormal muscle weakness and rapid fatigue.

Indications

Myditin is indicated for:

  • Myasthenia gravis: The primary indication. Myditin improves neuromuscular transmission and reduces muscle weakness in affected patients.
  • Postoperative intestinal atony (paralytic ileus): In selected cases, pyridostigmine may be used to support bowel motility after surgery.
  • Reversal of non-depolarizing neuromuscular blockade: Used in anaesthesiology to reverse the effects of certain muscle relaxants after surgical procedures.

Mechanism of Action

The active substance pyridostigmine bromide works by inhibiting the enzyme acetylcholinesterase. Under normal conditions, this enzyme breaks down the neurotransmitter acetylcholine after it has transmitted a nerve signal across the neuromuscular junction. By blocking this enzyme, Myditin allows acetylcholine to accumulate at the junction, prolonging and strengthening the signal to the muscle. This effectively compensates for the reduced number of functional acetylcholine receptors in patients with myasthenia gravis, thereby improving muscle strength and endurance.

Dosage and Administration

The dosage of Myditin is determined individually by the treating physician based on the severity of the condition and patient tolerance. General dosage guidance includes:

  • Tablets are taken orally, typically multiple times daily at regular intervals.
  • The usual adult daily dose for myasthenia gravis ranges from 30 mg to 360 mg, divided into several individual doses.
  • In children and elderly patients, doses are adjusted accordingly.
  • Consistent timing of doses is important to maintain stable drug levels and ensure continuous symptom control.

Side Effects

Because Myditin acts on the cholinergic system, typical cholinergic side effects may occur, particularly affecting the gastrointestinal tract, glands, and smooth muscle:

  • Common: Nausea, vomiting, diarrhoea, abdominal cramps, increased salivation, increased sweating
  • Occasional: Slow heart rate (bradycardia), low blood pressure, muscle twitching, miosis (narrowing of the pupils)
  • Rare: Bronchospasm, excessive bronchial secretion

Overdose can result in a cholinergic crisis, characterised by severe muscle weakness, excessive secretions, and respiratory distress, which requires immediate medical attention.

Contraindications

Myditin must not be used in patients with:

  • Mechanical obstruction of the gastrointestinal or urinary tract
  • Known hypersensitivity to pyridostigmine, bromides, or any of the excipients
  • Certain cardiac arrhythmias, particularly bradycardia

Drug Interactions

Myditin may interact with other medications. Key interactions include:

  • Non-depolarizing muscle relaxants: Their neuromuscular blocking effects may be reduced or reversed.
  • Beta-blockers and antiarrhythmics: May enhance bradycardic effects on the heart.
  • Antibiotics (e.g. aminoglycosides): Can impair neuromuscular transmission and reduce the effectiveness of Myditin.
  • Corticosteroids: May reduce the efficacy of pyridostigmine.

Important Information for Patients

Patients taking Myditin should be aware of the following:

  • Do not stop or adjust the dose without consulting a doctor, as this can lead to worsening of myasthenia gravis symptoms.
  • Regular medical check-ups are essential to monitor response to treatment and to detect side effects early.
  • Seek immediate medical attention if severe muscle weakness, breathing difficulties, or other serious symptoms develop.
  • Always inform surgeons, anaesthesiologists, and other healthcare providers about the use of Myditin before any planned procedure or surgery.
