What is Nafarelin?

Nafarelin is a synthetic analogue of the naturally occurring gonadotropin-releasing hormone (GnRH). Administered as a nasal spray, it acts on the pituitary gland to suppress the production of sex hormones such as estrogen and progesterone. Nafarelin is marketed under the brand name Synarel and belongs to the pharmacological class of GnRH agonists.

Indications

Nafarelin is used in the following clinical settings:

  • Endometriosis: Relief of pain and reduction of endometrial lesions in adult women.
  • Assisted Reproduction: Pituitary suppression as part of controlled ovarian stimulation protocols for in vitro fertilization (IVF).
  • Central Precocious Puberty: Management of early-onset puberty in children where the pituitary-gonadal axis is prematurely activated.

Mechanism of Action

Nafarelin is a chemically modified version of natural GnRH with a significantly longer half-life. With continuous administration, it causes downregulation and desensitization of pituitary GnRH receptors. As a result, the pituitary gland reduces its release of LH (luteinizing hormone) and FSH (follicle-stimulating hormone), which in turn leads to a marked suppression of gonadal sex hormone production. This creates a state of reversible hypogonadism – sometimes referred to as medical or pharmacological castration – that resolves after treatment is discontinued.

Initial Flare Effect

At the start of therapy, nafarelin initially stimulates a brief but significant surge in sex hormone levels, known as the flare effect. This transient increase may temporarily worsen symptoms such as endometriosis-related pain during the first one to two weeks of treatment.

Dosage and Administration

Nafarelin is administered as a nasal spray. For endometriosis, the standard dose is one spray into each nostril twice daily, providing a total daily dose of 400 micrograms. For central precocious puberty, higher doses may be required depending on body weight and clinical response. The standard treatment duration for endometriosis is 6 months. Extended use should be carefully evaluated due to the risk of progressive bone density loss.

Side Effects

Because nafarelin significantly reduces estrogen levels, it commonly produces menopausal-like symptoms:

  • Hot flashes and night sweats
  • Vaginal dryness and decreased libido
  • Mood changes and depressive symptoms
  • Headaches
  • Sleep disturbances
  • Decreased bone mineral density with prolonged use – an important monitoring parameter
  • Local nasal irritation from the spray formulation

Interactions and Important Notes

The following considerations are relevant during nafarelin therapy:

  • Nasal congestion (e.g., from a cold) may reduce drug absorption; a topical decongestant applied beforehand may be recommended.
  • Concurrent use of intranasal corticosteroids may interfere with nafarelin absorption.
  • Nafarelin is contraindicated during pregnancy and breastfeeding.
  • Non-hormonal contraception should be used during treatment, as nafarelin does not provide reliable contraceptive protection on its own.
