What does nasogastric mean?

The term nasogastric is derived from the Latin word for nose (nasus) and the Greek word for stomach (gaster). It describes a route or connection that runs from the nose through the throat and esophagus into the stomach. In clinical practice, this term is most commonly used in connection with the nasogastric tube.

The Nasogastric Tube

A nasogastric tube (NGT) is a thin, flexible tube that is inserted through one nostril, guided through the throat and esophagus, and placed into the stomach. It is one of the most widely used medical devices in hospitals and care facilities worldwide.

Indications and Uses

  • Enteral nutrition: Patients who are unable to swallow or eat independently (e.g., after a stroke, in cases of unconsciousness, or in premature infants) can be provided with liquid nutrition through the tube.
  • Gastric decompression: In cases of bowel obstruction or after certain surgical procedures, the tube can be used to drain stomach contents and gas, thereby reducing gastric pressure.
  • Medication administration: Liquid or dissolved medications can be delivered directly into the stomach via the tube.
  • Diagnostic purposes: Gastric samples can be collected through the tube, or contrast agents can be administered for imaging procedures.

How is a Nasogastric Tube Inserted?

Inserting a nasogastric tube is a routine medical procedure performed by trained nurses or physicians. The patient is positioned upright or with a slightly elevated head. The tube is lubricated and gently introduced through a nostril. The patient is asked to swallow to help guide the tube into the esophagus rather than the airway. After placement, the correct position in the stomach must be verified -- typically by an X-ray or by aspirating gastric contents and checking the pH level.

Possible Complications and Risks

Although a nasogastric tube is a safe and well-established medical tool, complications can occasionally occur:

  • Misplacement: The tube could accidentally be inserted into the trachea instead of the esophagus. This makes position verification mandatory before any use.
  • Mucosal irritation: The insertion process can irritate the mucous membranes of the nose, throat, and esophagus.
  • Aspiration: In rare cases, food or stomach contents may enter the lungs, potentially causing aspiration pneumonia.
  • Pressure injuries: With prolonged use, pressure sores may develop in the nasal mucosa or the back of the throat.
  • Tube blockage: Particularly with thick nutritional formulas, the tube can become blocked and may need to be flushed or replaced.

Care and Monitoring

Proper care of a nasogastric tube is essential for patient safety. This includes regular flushing of the tube, checking its position before each feeding, caring for the nasal mucosa, and securing the tube to the nose to prevent accidental dislodgement.
