Nasotracheal is a medical term describing a route or access pathway that passes through the nose (Latin: nasus) and continues into the trachea (windpipe). The term is most commonly used in the context of nasotracheal intubation, which involves inserting a breathing tube (endotracheal tube) through the nasal passage into the trachea to secure the airway.

Indications

Nasotracheal intubation is used in a variety of clinical situations, particularly when oral access is not feasible or desirable:

  • Surgical procedures involving the mouth, jaw, or face
  • Dental and oral surgery performed under general anesthesia
  • Trauma patients with limited mouth opening (trismus)
  • Long-term mechanical ventilation in intensive care when the oral route is unsuitable
  • Emergency airway management in specific cases of airway obstruction

Procedure

During nasotracheal intubation, a lubricated, flexible endotracheal tube is carefully inserted through one nostril. The tube is advanced through the nasopharynx (nasal throat), past the larynx (voice box), and into the trachea. The procedure is typically performed under direct vision using a laryngoscope or a flexible bronchoscope to ensure correct placement. Correct positioning is confirmed by auscultating breath sounds and using capnography (measurement of exhaled carbon dioxide).

  • Reduced risk of accidental tube displacement
  • Provides unobstructed access to the oral cavity during dental or oral surgery

Risks and Complications

As with any medical procedure, nasotracheal intubation carries certain risks and potential complications:

  • Nosebleeds (epistaxis) due to injury of the delicate nasal mucosa
  • Injury to the nasal sinuses or nasal cartilage
  • Pressure necrosis with prolonged tube placement
  • Sinusitis (inflammation of the sinuses) with long-term intubation
  • Misplacement of the tube into the esophagus instead of the trachea

Contraindications

Nasotracheal intubation should be avoided in certain situations:

  • Suspected basilar skull fracture
  • Severe coagulation disorders or coagulopathy
  • Nasal polyps or significant nasal obstruction
  • Known bleeding disorders
