What is NFP?
NFP stands for Natural Family Planning and refers to a group of methods that allow women to identify their fertile and infertile days within the menstrual cycle. These methods can be used either to avoid or to achieve a pregnancy – without the use of hormones or mechanical devices.
NFP is based on the systematic observation and recording of natural body signs that change throughout the menstrual cycle. The key indicators include basal body temperature, cervical mucus, and the position and texture of the cervix.
Methods of Natural Family Planning
Sympto-Thermal Method
The most widely used NFP method is the sympto-thermal method, which combines at least two body signs: the morning basal body temperature (resting body temperature) and the consistency of cervical mucus. This combination allows for precise identification of the fertile window.
Billings Ovulation Method
The Billings Ovulation Method relies solely on the observation of cervical mucus. During the fertile phase, the mucus changes – it becomes clearer, more stretchy, and slippery, similar to raw egg white. This change signals the approach of ovulation.
Basal Body Temperature Method
With the basal body temperature method, body temperature is measured each morning before getting up. After ovulation, the temperature rises by approximately 0.2–0.4 °C and remains elevated until the next menstruation. This rise indicates the infertile phase following ovulation.
Cycle Monitors and Apps
Modern cycle monitors and NFP apps support the recording and analysis of measured data. They simplify tracking and assist in interpreting the body signs. However, it is important that these digital tools are based on validated algorithms to ensure reliability.
How Reliable is NFP?
The effectiveness of NFP depends strongly on correct application and the experience of the user. When used correctly, the sympto-thermal method has a Pearl Index of 0.4–2.3, which is comparable in effectiveness to hormonal contraception. With imperfect use, the Pearl Index rises to approximately 20.
The Pearl Index indicates how many out of 100 women using a contraceptive method for one year experience an unintended pregnancy.

- Illness, travel, or sleep disturbances can affect measurement accuracy
Who Can Use NFP?
NFP is generally suitable for any woman who is willing to engage closely with her cycle. A course with a certified NFP counselor is strongly recommended to ensure safe and effective application. Professional guidance significantly improves method reliability.
Women with very irregular cycles, certain medical conditions, or those in hormonally unstable life phases should consult a healthcare provider or qualified counselor before using NFP.
NFP and Fertility
NFP is not only used for contraception but also to actively support conception. By accurately identifying fertile days, couples can time intercourse to maximize the chances of pregnancy. This makes NFP a valuable tool for those trying to conceive.
