Normocaloric FSMP are characterized by an energy density of approximately 1 kcal per milliliter. Compared to high-calorie products (1.5–2 kcal/ml), they provide a moderate caloric intake at a standard fluid volume. They are particularly suitable for individuals who do not require fluid restriction and whose primary goal is complete or supplementary nutritional support.

Legal Framework and Definition

In the European Union, FSMP are regulated by Regulation (EU) No. 609/2013 and Commission Delegated Regulation (EU) 2016/128. These products may only be used under medical supervision or on the advice of a qualified nutrition professional. They must meet strict requirements regarding composition, labeling, and safety.

Indications and Areas of Use

Normocaloric FSMP are used in a wide range of medical situations, including:

  • Malnutrition and cachexia associated with chronic illness
  • Dysphagia (swallowing difficulties) following stroke or in neurological conditions
  • Oncological diseases where normal food intake is compromised
  • Gastrointestinal disorders such as Crohn's disease, short bowel syndrome, or inflammatory bowel disease

Oral Nutritional Supplements (ONS)

Normocaloric FSMP are frequently available as oral nutritional supplements (ONS), taken in addition to normal food when dietary intake alone is insufficient. These products are offered in various flavors and consistencies to improve acceptance and compliance.

Tube Feeding

Normocaloric FSMP can also be administered as tube feeding via nasogastric tube, percutaneous endoscopic gastrostomy (PEG), or jejunal tube when oral intake is not possible. Administration is typically carried out continuously via a feeding pump or as bolus feeding.

Composition and Nutritional Profile

Normocaloric FSMP are generally nutritionally complete, providing all essential nutrients in adequate amounts:

  • Macronutrients: Carbohydrates, proteins, and fats in balanced proportions
  • Micronutrients: Vitamins (e.g., vitamin D, vitamin B12, folic acid) and minerals (e.g., calcium, magnesium, zinc)
  • Trace elements: Selenium, iron, iodine, and others
  • Dietary fiber: Many products contain soluble and insoluble fiber to support gut health

Specialized variants are adapted for specific conditions, such as protein-enriched normocaloric FSMP for patients with elevated protein requirements, or formulas with modified carbohydrate content for patients with diabetes.

Difference Between Normocaloric and High-Calorie FSMP

While normocaloric FSMP provide approximately 1 kcal per ml, high-calorie products contain 1.5 to 2 kcal per ml. Normocaloric products are therefore suitable when no fluid restriction is required and caloric needs can be met at a standard volume. High-calorie FSMP are preferred in cases of fluid restriction (e.g., renal or cardiac insufficiency) or when energy demands are particularly high.

Usage and Dosage

The dosage and duration of normocaloric FSMP use depend on individual nutritional status, energy requirements, and underlying medical conditions. Guidance from a physician and a qualified nutrition professional is strongly recommended. As a general reference:

  • As oral nutritional supplements: 1 to 3 portions per day (typically 200–300 ml each) in addition to regular meals
  • As tube feeding: the required daily amount is individually calculated and administered continuously or as bolus doses

Tolerability and Possible Side Effects

Normocaloric FSMP are generally well tolerated. Possible complaints during the initiation of nutritional therapy may include:

  • Nausea or feeling of fullness, especially at the beginning
  • Diarrhea or constipation (depending on fiber content and route of administration)
  • Bloating or flatulence

These issues can often be minimized by gradually increasing the dose or switching to a different product. Medical advice should be sought in cases of intolerance, allergic reactions, or persistent complaints.
