The OPQRST schema is a standardized assessment mnemonic used across clinical medicine, emergency care, and nursing to systematically gather information about a patient's pain or discomfort. Each letter of the acronym represents a specific line of questioning that helps healthcare professionals build a complete clinical picture of the presenting complaint. The tool forms an essential part of medical history taking (anamnesis) and is widely used in hospitals, emergency services, and primary care settings worldwide.
What Does Each Letter Stand For?
The acronym OPQRST breaks down as follows:
- O – Onset: When did the symptoms begin? Did they come on suddenly or gradually? What was the patient doing when they first appeared?
- P – Provocation / Palliation: What makes the pain worse? What makes it better? Are there specific movements, positions, or activities that influence the symptoms?
- Q – Quality: How would the patient describe the pain? Is it sharp, dull, burning, pressing, cramping, or throbbing? The quality of pain often points toward specific underlying causes.
- R – Region / Radiation: Where exactly is the pain located? Does it radiate to other areas of the body, such as from the chest to the left arm or jaw?
- S – Severity: How intense is the pain on a scale from 0 to 10? This numerical rating allows for objective monitoring of changes over time.
- T – Time / Timing: How long have the symptoms been present? Are they constant or intermittent? Have they changed in character or intensity over time?
When is the OPQRST Schema Used?
The OPQRST schema is applied whenever a patient reports pain or other physical complaints. Common clinical settings include:
- Emergency departments and pre-hospital care: Rapid, structured assessment of acute conditions such as chest pain, abdominal pain, or headache.
- General practice and specialist consultations: Systematic evaluation of chronic or recurring complaints.
- Hospital wards and nursing care: Documentation, patient handovers, and care planning.
- First responders and paramedics: Structured communication during emergency response.
Advantages of the OPQRST Schema
The primary strength of the OPQRST schema lies in its simplicity and comprehensiveness. By following a consistent structure, clinicians are less likely to overlook important details. The schema:

A commonly paired tool is the SAMPLE schema (Symptoms, Allergies, Medications, Past history, Last meal, Events), which captures additional background information about the patient's medical history. Together, OPQRST and SAMPLE provide a comprehensive foundation for both pre-hospital and in-hospital patient assessment.
