Orthodontics is a recognized specialty within dentistry that deals with the diagnosis, prevention, and treatment of dental and jaw misalignments, known as malocclusions. Orthodontists complete several years of postgraduate specialty training after dental school.

The aim of orthodontic treatment goes beyond cosmetic improvement. It also focuses on restoring or optimizing chewing function, speech, and long-term oral health.

Common Indications

Orthodontic treatment is recommended for a variety of dental and skeletal conditions, including:

  • Crowding: Insufficient space in the jaw causes teeth to overlap or rotate.
  • Overbite (deep bite): The upper front teeth excessively cover the lower front teeth.
  • Underbite: The lower teeth protrude in front of the upper teeth.
  • Crossbite: Upper and lower teeth do not align correctly from side to side.
  • Open bite: A gap exists between the upper and lower front teeth even when the mouth is closed.
  • Spacing: Excessive gaps between teeth.

Causes of Dental Misalignment

Malocclusions develop due to a combination of genetic predisposition and environmental factors:

  • Hereditary factors (e.g., small jaw with large teeth)
  • Prolonged thumb-sucking or pacifier use in early childhood
  • Early loss of baby teeth
  • Mouth breathing due to chronic nasal obstruction
  • Abnormal tongue or lip posture habits
  • Injuries or diseases affecting the jawbone

Diagnosis

Orthodontic diagnosis involves a comprehensive assessment of the teeth, jaws, and facial structure, including:

  • Clinical examination: Visual inspection of teeth, dental arches, and bite relationships
  • X-rays: Panoramic radiographs and lateral cephalometric X-rays to evaluate jaw position and bone growth
  • Dental models: Physical impressions or digital scans of the dental arches for treatment planning
  • Photo documentation: Facial and intraoral photographs for monitoring progress

Treatment Methods

Removable Appliances

Removable braces (plates) are commonly used during the growth phase in children to guide jaw development and correct mild misalignments. Functional appliances such as the activator use muscle forces to influence jaw growth.

Fixed Appliances

The fixed multi-bracket appliance (traditional braces with brackets and wires) is the most widely used fixed orthodontic device. It allows precise three-dimensional tooth movement and is used for complex malocclusions. Brackets can be made of metal, ceramic, or plastic.

Aligner Therapy

Clear aligners (e.g., Invisalign) are removable, nearly invisible plastic trays that gradually move teeth into the desired position. They are particularly popular among adult patients and are suitable for mild to moderate misalignments.

Surgical Orthodontics

In cases of severe skeletal discrepancies that cannot be corrected by tooth movement alone, orthodontic treatment is combined with orthognathic surgery. This procedure surgically repositions the upper jaw, lower jaw, or both to achieve proper alignment.

Treatment Duration and Retention

The active treatment phase typically lasts between one and three years, depending on the severity of the condition. After active treatment, a retention phase is essential: retainers -- either removable retainer trays or fixed bonded wires -- prevent teeth from drifting back to their original positions, a process known as relapse.

Orthodontics for Children, Teenagers, and Adults

The optimal time for treatment is often during the growth phase, between the ages of 9 and 14, when jaw growth can be actively utilized. However, orthodontic treatment is possible at any age. Adults increasingly favor discreet options such as ceramic brackets or clear aligners.
