Oxymetazoline is an alpha-adrenergic sympathomimetic agent primarily used as a topical nasal decongestant. It belongs to the class of vasoconstrictive substances and is available over the counter under various brand names. The drug is applied locally and acts directly on the mucosal blood vessels of the nose, and in certain formulations, the eyes.

Oxymetazoline acts as a selective alpha-2 adrenoceptor agonist and, at higher concentrations, also as an alpha-1 adrenoceptor agonist. Activation of these receptors in the walls of the nasal mucosal blood vessels causes vasoconstriction -- a narrowing of the small blood vessels. This reduces blood flow to the swollen mucosa, decreases mucosal edema, and typically restores free nasal breathing within minutes. The effect generally lasts 8 to 12 hours.

The recommended duration of use is no more than 3 to 7 days, as prolonged use can lead to a rebound effect known as rhinitis medicamentosa. Typically, 1 to 2 sprays per nostril, 2 to 3 times daily, are recommended. Oxymetazoline should not be used in children under 2 years of age.
