What is P-Selectin?
P-Selectin (also known as CD62P, GMP-140, or PADGEM) is a cell surface protein belonging to the selectin family. It is primarily stored in platelets and endothelial cells and is rapidly transported to the cell surface upon their activation. P-Selectin plays a critical role in inflammatory responses, blood coagulation, and various pathological processes such as atherosclerosis and thrombosis.
Biological Function and Mechanism of Action
P-Selectin belongs to the group of adhesion molecules and mediates the initial contact between immune cells (leukocytes) and the vessel wall. This process, known as leukocyte rolling, is an early step in the inflammatory response.
- Storage: In platelets, P-Selectin is stored in alpha granules; in endothelial cells, it is stored in Weibel-Palade bodies.
- Activation: Upon injury or inflammation, these stores are released and P-Selectin appears on the cell surface within minutes.
- Binding: P-Selectin binds to its ligand PSGL-1 (P-Selectin Glycoprotein Ligand-1), which is expressed on the surface of leukocytes and monocytes.
- Leukocyte Recruitment: This binding slows leukocytes in the bloodstream and enables their migration into inflamed tissue.
Clinical Significance
Thrombosis and Cardiovascular Disease
Elevated P-Selectin levels in the blood are associated with an increased risk of thrombosis, myocardial infarction, and stroke. Since P-Selectin promotes the adhesion of leukocytes to platelets, it plays an important role in the formation of blood clots (thrombi).
Atherosclerosis
In atherosclerosis (hardening of the arteries), P-Selectin is involved in the migration of immune cells into the vessel wall, contributing to plaque formation and arterial narrowing.
Inflammatory Diseases
In chronic inflammatory conditions such as rheumatoid arthritis, inflammatory bowel disease (Crohn's disease, ulcerative colitis), and bronchial asthma, P-Selectin is overactivated and contributes to tissue damage.
Sickle Cell Disease
In sickle cell disease, P-Selectin plays a particularly important role: the abnormally shaped red blood cells activate the endothelium, leading to overexpression of P-Selectin and triggering vascular occlusions known as vaso-occlusive crises.
P-Selectin as a Diagnostic Marker
Soluble P-Selectin (sP-Selectin) can be measured in blood plasma and serves as a biomarker for platelet and endothelial cell activation. Elevated levels are found in:
- Acute myocardial infarction
- Deep vein thrombosis (DVT)
- Pulmonary embolism
- Diabetic angiopathy
- Systemic lupus erythematosus (SLE)
P-Selectin as a Therapeutic Target
Due to its central role in inflammation and thrombosis, P-Selectin is an important target for new medications. Crizanlizumab (brand name: Adakveo) is a monoclonal antibody that inhibits P-Selectin and is used to prevent vaso-occlusive crises in sickle cell disease. Further P-Selectin inhibitors are in clinical development for other indications such as thrombosis and inflammatory diseases.
