What is Palliation?

Palliation refers to all medical, nursing, and psychosocial measures aimed at relieving suffering and improving comfort in patients with serious, life-threatening, or incurable diseases. The focus is not on curing the disease but on enhancing the quality of life of the patient and their loved ones. Palliation is an essential part of modern medicine and can be applied at any stage of a serious illness – not only at the end of life.

Goals of Palliation

The core goals of palliation include:

  • Relief from pain and other distressing symptoms such as breathlessness, nausea, and fatigue
  • Preserving the dignity and autonomy of the patient
  • Addressing psychological, social, and spiritual needs
  • Supporting family members during illness and bereavement
  • Fostering open communication between the patient, family, and care team

Palliation is used across a wide range of conditions, including:

  • Cancer (oncology)
  • Advanced cardiovascular diseases
  • End-stage chronic obstructive pulmonary disease (COPD)
  • Neurodegenerative diseases such as ALS or Parkinson disease

  • Advanced HIV/AIDS

Palliative Treatment Measures

Pain Management

Pain management is a cornerstone of palliation. It commonly follows the WHO analgesic ladder, ranging from non-opioid analgesics to strong opioids. The aim is adequate pain relief with minimal side effects.

Symptom Control

Beyond pain, other symptoms are actively managed, including breathlessness through oxygen therapy or opioids, nausea through antiemetics, and fatigue through tailored activity and rest planning.

Psychosocial and Spiritual Support

Palliation also encompasses psychological care for patients and families. Social workers, chaplains, and specialist palliative nurses work together as a team to support the whole person.

Palliative Care Units and Home-Based Care

Palliative patients may be cared for in dedicated palliative care units in hospitals, in hospices, or at home. Specialist palliative home care services enable high-quality palliative support in familiar surroundings.

Palliation vs. Curative Treatment

Palliation and curative treatment are not mutually exclusive. In many cases, both approaches are pursued in parallel. When curative measures are no longer appropriate, palliation takes priority to ensure the patient can live with dignity until the end of life.

Relevance and Societal Importance

With an aging population and a growing burden of chronic disease, palliation is becoming increasingly important. Well-organized palliative care not only relieves patients and their families but also contributes to healthcare efficiency by avoiding unnecessary and burdensome treatments.
