It works by blocking the enzyme system responsible for producing stomach acid, making it effective for treating conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, duodenal ulcers, and for preventing ulcers caused by non-steroidal anti-inflammatory drugs (NSAIDs). It is available as oral tablets and as an intravenous infusion and is one of the most widely prescribed medications in the world.
Common Side Effects
Most side effects of pantoprazole are mild and resolve after stopping the medication. Common side effects (affecting 1 in 10 to 1 in 100 people) include:
- Headache
- Diarrhea or loose stools
- Constipation
- Flatulence and abdominal pain
- Nausea and vomiting
- Dizziness
- Elevated liver enzyme levels (usually temporary)

- Interstitial nephritis: A rare inflammation of the kidneys.
- Stevens-Johnson syndrome or toxic epidermal necrolysis: Very rare, severe skin reactions.
Long-Term Side Effects
Patients taking pantoprazole long-term (over several months or years) should be aware of the following:
- Regular monitoring of blood magnesium levels, especially if also taking diuretics or digoxin.
- Monitoring of vitamin B12 status, as chronically reduced stomach acid can impair its absorption.
- Assessment of bone density in elderly patients or those at risk of osteoporosis.
- Periodic review of whether continued use is necessary and whether a gradual dose reduction is appropriate.
Drug Interactions
Pantoprazole may affect the efficacy of certain other medications:
- Clopidogrel: Pantoprazole may reduce the effectiveness of this antiplatelet drug.

- Ketoconazole and itraconazole: Reduced absorption of these antifungal agents due to lower stomach acidity.
- Atazanavir (HIV medication): Significantly reduced absorption when taken together.
