- Pathognomonic sign: Occurs only in a single disease and allows for a definitive diagnosis.
- Characteristic sign: Frequently associated with a particular disease but can also appear in other conditions.
- Non-specific symptom: Can occur in many different diseases, such as fever or fatigue.
Examples in Medicine
Truly pathognomonic findings are uncommon in clinical medicine. Among the most well-known examples are:
- Koplik spots: Small whitish spots on the oral mucosa that occur exclusively in measles and are considered pathognomonic for this disease.
- Aschoff nodules: Histological granulomas considered pathognomonic for rheumatic fever.
- Reed-Sternberg cells: Distinctive giant cells found in tissue biopsies that are pathognomonic for Hodgkin lymphoma.
- Kernig and Brudzinski signs: Strongly associated with meningitis, though not always classified as strictly pathognomonic.
Pathognomonic Findings in Diagnostics
In modern medicine, the term is sometimes used more broadly to include findings that are not entirely exclusive to one disease but are so strongly associated with a specific condition that they are considered diagnostically conclusive. This may apply to imaging findings, laboratory values, or histological results.
The identification of pathognomonic signs is a central goal in clinical diagnostics, as it significantly streamlines the diagnostic process. When no such definitive sign is present, a differential diagnostic workup is required, systematically ruling out various possible conditions.
Relevance for Patients
For patients, it is reassuring to know that a pathognomonic finding provides a high degree of diagnostic certainty. When a physician identifies such a sign, a reliable diagnosis can be made quickly, enabling prompt and targeted treatment. In most clinical situations, however, multiple diagnostic steps are still required before a final diagnosis is confirmed.
