Plasmodium knowlesi is a single-celled parasitic organism belonging to the genus Plasmodium, which causes malaria. Unlike most malaria parasites that exclusively infect humans, P. knowlesi is a zoonotic pathogen – meaning it primarily circulates in animal hosts (particularly macaque monkeys) and can be transmitted to humans. It is now recognized as the fifth species of Plasmodium capable of causing malaria in humans.
Plasmodium knowlesi is predominantly found in Southeast Asia, particularly in:
Transmission to humans occurs through the bite of infected Anopheles mosquitoes, especially Anopheles latens and related species. These mosquitoes typically inhabit forested areas and transmit the parasite after feeding on infected long-tailed macaques (Macaca fascicularis). There is currently no confirmed evidence of direct human-to-human transmission.
The incubation period is typically 9 to 12 days after a mosquito bite. Unlike other malaria parasites, P. knowlesi completes its blood-stage replication cycle every 24 hours (quotidian fever), meaning fever episodes can occur daily.
Common symptoms include:
Similar to Plasmodium falciparum, P. knowlesi can cause severe disease, including:
Diagnosing Plasmodium knowlesi infection can be challenging because the parasite closely resembles other species under the microscope, particularly P. malariae. The following diagnostic methods are used:
Treatment of Plasmodium knowlesi malaria depends on disease severity and follows established malaria management guidelines:
Early diagnosis and treatment are essential to prevent life-threatening complications.
Since P. knowlesi is transmitted by Anopheles mosquitoes, the following protective measures are recommended:
Plasmodium knowlesi is the leading cause of malaria in Malaysia and represents a growing public health concern across Southeast Asia. Deforestation and the resulting increased contact between humans, monkeys, and mosquitoes are key drivers of transmission. Travelers to affected regions should be informed about the risk of infection.
