What is Plasmodium vivax?
Plasmodium vivax is a single-celled parasitic organism belonging to the genus Plasmodium and is the most widespread cause of vivax malaria (also called tertian malaria). Although generally considered less deadly than malaria caused by Plasmodium falciparum, infections with P. vivax are particularly challenging because the parasite can form dormant liver stages known as hypnozoites, which can reactivate weeks, months, or even years later, causing relapses.
Distribution and Transmission
Plasmodium vivax is found across a wide geographic range, including South Asia, Southeast Asia, Latin America, and parts of Africa. Transmission occurs primarily through the bite of an infected female Anopheles mosquito. Less commonly, the parasite can be transmitted via blood transfusions, shared needles, or from mother to child during pregnancy.
Life Cycle of the Parasite
The life cycle of P. vivax involves several distinct stages:
- Sporozoites are injected into the human bloodstream during a mosquito bite and travel to the liver.
- In the liver, they develop into merozoites, which are released into the bloodstream and invade red blood cells.
- Some liver-stage parasites remain dormant as hypnozoites, which can be reactivated later and cause relapses.
- Inside red blood cells, the parasites replicate every 48 hours, causing the characteristic fever spikes that occur every two to three days.
Symptoms
The clinical symptoms of a P. vivax infection include:
- Recurring fever every 48 hours (tertian fever pattern)
- Chills and profuse sweating
- Headache and muscle aches
- Fatigue and general malaise
- Enlarged spleen (splenomegaly) in prolonged cases
- Pallor and anemia due to destruction of red blood cells
Severe complications are less common than with P. falciparum but can occur, particularly in young children, pregnant women, or immunocompromised individuals.
Diagnosis
Diagnosis of a Plasmodium vivax infection is confirmed through:
- Microscopic blood smear: The gold standard method, allowing direct visualization of the parasite in blood samples.
- Rapid Diagnostic Tests (RDTs): Antigen-based tests that provide quick results in resource-limited settings.
- Polymerase Chain Reaction (PCR): A highly sensitive method used to confirm species identification.
Treatment
Treatment targets both the blood-stage parasites and the dormant liver stages:
- Chloroquine: The first-line treatment for blood-stage infection in regions where P. vivax remains sensitive to this drug.
- Primaquine: Used in addition to chloroquine to eliminate hypnozoites and prevent relapses. Before prescribing, patients must be tested for G6PD deficiency (glucose-6-phosphate dehydrogenase deficiency), as primaquine can trigger life-threatening hemolytic anemia in affected individuals.
- Tafenoquine: A newer single-dose alternative to primaquine for the prevention of relapse, also requiring G6PD screening prior to use.
Prevention
Preventive measures against P. vivax infection include:
- Using insect repellents containing DEET or picaridin
- Sleeping under insecticide-treated bed nets
- Wearing long-sleeved clothing during dusk and nighttime hours
- Taking antimalarial prophylaxis when traveling to endemic regions
