Things worth knowing about "Pneumonic plague"

Pneumonic plague is a life-threatening bacterial lung infection caused by Yersinia pestis. It is considered the most dangerous form of plague.

What is Pneumonic Plague?

Pneumonic plague is the most severe and rapidly progressing form of plague. It is caused by the bacterium Yersinia pestis and primarily affects the lungs. Without prompt treatment, the disease is fatal in nearly all cases. Pneumonic plague is one of three main forms of plague, alongside bubonic plague and septicemic plague.

Causes and Transmission

Pneumonic plague is caused by Yersinia pestis, a gram-negative rod-shaped bacterium. There are two forms of transmission:

  • Primary pneumonic plague: Occurs through direct inhalation of infectious respiratory droplets from a sick person or animal. This form is highly contagious and can spread from person to person.
  • Secondary pneumonic plague: Develops as a complication of untreated bubonic or septicemic plague, when bacteria spread through the bloodstream to the lungs.

Natural reservoirs include rodents such as rats and prairie dogs. The pathogen is typically transmitted to humans via flea bites, but pneumonic plague can also spread directly between people.

Symptoms

The incubation period of pneumonic plague is only 1 to 4 days. The illness begins suddenly and progresses rapidly:

  • Sudden onset of high fever
  • Severe malaise and chills
  • Intense cough with bloody or mucous sputum
  • Shortness of breath and chest pain
  • Headache and general weakness
  • In advanced stages: confusion, circulatory failure, respiratory failure

Without treatment, pneumonic plague leads to death within 2 to 4 days of symptom onset.

Diagnosis

Diagnosis requires rapid identification of Yersinia pestis. The following diagnostic methods are used:

  • Microscopy and culture: Examination of sputum, blood, or lymph node fluid for the presence of the bacterium.
  • PCR (polymerase chain reaction): A rapid and highly specific method for detecting the genetic material of the pathogen.
  • Antigen tests: Detection of specific surface proteins of Yersinia pestis.
  • Chest X-ray or CT scan: Reveals typical consolidations and signs of inflammation in the lungs.

Due to the rapid course of the disease, treatment must be started immediately upon clinical suspicion, even before laboratory results are available.

Treatment

Pneumonic plague is a medical emergency. Immediate treatment with antibiotics is life-saving:

  • Streptomycin or gentamicin are considered the first-line treatments.
  • Alternatives include doxycycline, ciprofloxacin, or chloramphenicol.
  • The typical treatment duration is 10 to 14 days.

Patients must be placed under strict isolation to prevent further transmission. Close contacts receive prophylactic antibiotics. In severe cases, intensive care with respiratory support may be required.

Historical Significance and Global Occurrence

Plague has caused several devastating pandemics throughout history, most notably the Black Death in the 14th century, which is estimated to have killed one third of the European population. Today, plague cases still occur sporadically in parts of Africa, Asia, and North America. The World Health Organization (WHO) reports several hundred cases annually worldwide. With modern antibiotics, the disease is treatable when diagnosed early.

Prevention

  • Avoid contact with rodents and fleas in endemic areas
  • Use of insect repellents
  • Use of personal protective equipment by healthcare workers
  • Prophylactic antibiotics for confirmed close contacts
  • A widely available licensed vaccine for the general public is currently not available
