Preconception counseling (also called preconception care) is a medical consultation that takes place before a planned pregnancy. It is aimed at women and couples who wish to have a child, with the goal of creating the best possible conditions for a healthy pregnancy and a healthy baby. The focus is on identifying individual risk factors, optimizing overall health, and providing targeted guidance on important measures to take before conception.
Preconception counseling pursues several important objectives:
The consultation begins with a thorough medical history. This covers chronic illnesses, previous pregnancies, miscarriages, hereditary conditions in the family, and current medications. A physical examination and, if needed, laboratory tests (such as blood count, blood type, hormone levels, and infectious disease serology) complete the assessment.
One of the most important recommendations in preconception counseling is the timely intake of folic acid. The World Health Organization (WHO) and national health authorities recommend taking at least 400 micrograms of folic acid daily, starting at least four weeks before conception and continuing through the first trimester, to prevent neural tube defects in the baby. Supplementation with iodine is also recommended, as iodine deficiency can impair thyroid function and the brain development of the child.
The vaccination status of the woman is reviewed during the consultation. Certain infectious diseases such as rubella or chickenpox (varicella) can cause severe harm to the unborn child if contracted during pregnancy. Any missing vaccinations should ideally be administered before conception, as live vaccines are contraindicated during pregnancy.
Women with chronic conditions such as diabetes mellitus, epilepsy, high blood pressure, or thyroid disorders require particularly careful preconception counseling. Some medications are not safe during pregnancy and may need to be switched to safer alternatives. Achieving optimal control of existing conditions before conception is also critical for a healthy pregnancy outcome.
A healthy lifestyle plays a central role in fertility and the course of a pregnancy. The counseling session addresses the following factors:
Couples with known hereditary conditions, consanguineous couples (those who are related by blood), or individuals who have experienced repeated miscarriages are advised to seek genetic counseling. This involves assessing potential risks for the child and discussing available diagnostic options.
In principle, any woman or couple planning a pregnancy can benefit from preconception counseling. It is especially recommended for:
Preconception counseling is typically provided by gynecologists and obstetricians. Depending on individual needs, other specialists such as internists, endocrinologists, or clinical geneticists may also be involved. In some cases, a general practitioner or midwife can also provide initial preconception guidance.
