Prolaktin ist ein Hormon, das in der Vorderlappen-Hypophyse (Adenohypophyse) produziert wird und vor allem für die Milchbildung (Laktation) nach der Geburt verantwortlich ist.
Prolaktin gehört zur Familie der Peptidhormone und wird durch den Hypothalamus über das Neurotransmitter-System (insbesondere durch Dopamin) reguliert. Die Ausschüttung von Prolaktin steigt besonders in der Schwangerschaft und während des Stillens, wenn mechanische Reize wie das Saugen des Säuglings die Hormonfreisetzung stimulieren.
Neben seiner zentralen Rolle in der Laktation hat Prolaktin auch vielfältige Wirkungen im Körper. Es beeinflusst das Immunsystem, den Wasser- und Elektrolythaushalt, die Reproduktionsfunktion sowie das Verhalten. Bei Frauen kann ein dauerhaft erhöhter Prolaktinspiegel (Hyperprolaktinämie) zu Zyklusstörungen, Unfruchtbarkeit oder Galaktorrhoe (Milchfluss außerhalb der Stillzeit) führen. Bei Männern äußert sich ein erhöhter Spiegel häufig in Libidoverlust, Potenzstörungen und verminderter Testosteronproduktion.
Die Regulation von Prolaktin erfolgt vor allem über Dopamin, das hemmend auf seine Ausschüttung wirkt. Ursachen einer Hyperprolaktinämie können Stress, bestimmte Medikamente (z. B. Antidepressiva), Hypophysenadenome oder Hypothyreose sein. Zur Diagnostik werden Serumspiegelmessungen durchgeführt, und bei pathologisch erhöhten Werten kann eine medikamentöse Behandlung mit Dopaminagonisten (z. B. Cabergolin) erforderlich sein.
