What Is the PSA Level?

The PSA level (Prostate-Specific Antigen) is a blood test result that measures the concentration of a protein produced exclusively by the prostate gland. It is primarily used in medicine for the early detection of prostate cancer and for monitoring other prostate conditions. An elevated PSA level can indicate various prostate disorders but is not on its own proof of cancer.

When Is the PSA Test Used?

The PSA test is commonly used in the following situations:

  • Early detection (screening) of prostate cancer
  • Monitoring the progression of known prostate cancer
  • Follow-up after prostate cancer treatment
  • Investigation of urinary symptoms or suspected prostate conditions

Normal Values and Interpretation

PSA levels are measured in nanograms per milliliter of blood (ng/ml). General reference ranges are:

  • Below 4 ng/ml: Generally considered normal (age-dependent)
  • 4 to 10 ng/ml: Grey zone, further evaluation recommended
  • Above 10 ng/ml: Elevated risk, urgent further diagnostics required

It is important to note that normal ranges vary with age. Younger men typically have lower PSA levels than older men. Many urologists therefore apply age-adjusted reference values when interpreting results.

Causes of an Elevated PSA Level

A high PSA level does not necessarily mean prostate cancer. Possible causes include:

  • Benign prostatic hyperplasia (BPH): A non-cancerous enlargement of the prostate
  • Prostatitis: Inflammation or infection of the prostate
  • Prostate cancer (prostate carcinoma)
  • Physical activities such as cycling shortly before the test
  • Sexual activity shortly before blood collection
  • Prostate procedures or urinary catheterization

Diagnosis and Further Investigations

If the PSA level is elevated, a doctor will typically recommend further tests, such as:

  • Digital rectal examination (DRE): Manual palpation of the prostate through the rectum
  • Transrectal ultrasound (TRUS): Ultrasound imaging of the prostate
  • Prostate biopsy: Collection of tissue samples for microscopic analysis
  • MRI of the prostate: Detailed imaging to assess suspicious areas

Benefits and Limitations of the PSA Test

The PSA test is a useful screening tool, but it has limitations. It can produce false-positive results (elevated level without cancer) or false-negative results (normal level despite cancer being present). For this reason, the test result is always assessed in conjunction with other findings and the individual medical history of the patient. Most medical guidelines recommend discussing the test with a healthcare professional and making an informed, shared decision.

Treatment for an Elevated PSA Level

Treatment depends on the underlying cause:

  • For prostatitis: Antibiotics and anti-inflammatory medications
  • For BPH: Medications (alpha-blockers, 5-alpha-reductase inhibitors) or surgical procedures
  • For prostate cancer: Active surveillance, surgery (prostatectomy), radiation therapy, hormone therapy, or chemotherapy depending on the stage
